5 HMIS Homeless Management Information System Shelter Policy Board is a coalition of homeless serving organizations administratively housed in United Way System was mandated by HUD Proprietary product of Bowman Internet Systems Used by multiple non-profit organizations in the homeless serving agencies

6 HIEx™ HealthLink Information Exchange HealthLink is a coalition of health related organizations administratively housed at the Center for Healthy Communities at Wright State University Cross sector (health & human services) Household record for eligibility & means tested programs Patient centric health record Open source philosophy

7 Data sharing Proof of concept is the first step Create a unique identifier for all individuals from an algorithm Apply that algorithm to the data in all 3 systems Potential of interoperability through web services

9 Results from HIEx™ Most of the 26,000 individuals with data in HIEx™ live in 9600 households. The focus of the program has been to improve access to health care for families and children. At least 10% of children enrolled in Dayton Public Schools already have data in HIEx™
