3 Why a Survey?  To better understand the needs and wants in the TownCreek Community.  To quantify the needs in the community – DATA!  THE GOAL: is to use this data to create a community action plan to help address the challenges, found in the survey, through community programing. BHO 8/31/2015

4 Community Survey Info  Began Community Survey in October 2014.  TownCreek Vision distributed 300 surveys in churches and at community center.  TownCreek had a community action day and passed out surveys.  TownCreek Vision collected surveys in May 2015.  68 surveys were collected.  23% completion rate.  Average completion rate is between 10% - 15%. BHO 8/31/2015

12 3 Important Takeaways  Lots of woman participation.  Many educated community members.  Opportunity to better engage Hispanics/Latinos and the millennial generation. BHO 8/31/2015

38 3 Important Takeaways  Diabetes and Hypertension are the number 2 health concerns in the community.  Less fast food and more fruits and vegetables.  Being Healthy is a lifestyle that can be integrated in each program. BHO 8/31/2015
