Seattle Maritime Academy Acetylene Cylinder Response Ballard, WA – July 17, 2014  King County Hazardous Waste Program referral to EPA, request for assistance from Seattle Maritime Academy  Rusted compressed gas cylinder of acetylene gas. Deemed unstable, vendor refused to remove it  Next to residential area, highway  EPA and START response Air monitoring and safety Air monitoring and safety  EPA arranged for Air Gas local Emergency Response Team to assess and dispose of cylinder

Fred Meyer Propylene Glycol Spill Clackamas, OR – Aug 21, 2014  Sub-contractors working on fire suppression system poured 120 gallons propylene glycol into storm drain  Storm drain led to Carli Creek, flows to Clackamas River  FM secured discharge, hired contractor to remove product and assess creek  Technical assistance and direction provided to FM by on-call Phone Duty Officer (OSC)

7. 7 Magnolia Bridge UPRR Bakken Derailment Seattle, WA – July 24  BNSF Oil Tank Car Derailment, 4 cars derailed at Interbay rail yard, 2:00 AM, under Magnolia Bridge near Elliott Bay  No discharge of oil, potential threat to Smith Cove  EPA mobilization, assessment, coordination with WA Ecology  BNSF Hazmat, SFD, BNSF Haz contractors on-scene  Railcars up-righted with no further incident

Bonanza Mine & Mill Removal Sutherlin, OR  Abandoned mercury mine, operated 1860’s to 1960.  Second largest mercury mine in U.S., Over 3,000,000 lbs liquid mercury produced Over 3,000,000 lbs liquid mercury produced 12 adits, > 3 miles subterranean tunnels and shafts 12 adits, > 3 miles subterranean tunnels and shafts  Threat - high levels of mercury (various forms) and arsenic distributed over many acres in calcine (retorted ore), waste rock, tailings, soil, surrounding hillside, valley floor, roadways  EPA removal of wastes, consolidation & placement into on- site, lined repository. Over twice estimated volumes  4 generations of families living on site – moved to temporary housing by ODEQ. House trailers disposed of, EPA providing replacement house trailers & re-establishing new residence locations in clean areas.  EPA removal costs estimated at $3,215,000 10
