1 1 PROJECTS FOR ASSISTANCE IN TRANSITION FROM HOMELESSNESS (PATH)

2 2 PATH PROGRAM Created under the McKinney Homeless Assistance Amendments Act of 1990 Formula grant program that funds the 50 States and the territories Supports service delivery to individuals with serious mental illnesses, as well as individuals with co-occurring substance use disorders, who are homeless or at risk of becoming homeless

3 3 PATH PROGRAM DEFINITIONS Homeless Individual – an individual who lacks housing, including an individual whose primary residence during the night is a supervised public or private facility that provides temporary living accommodations

4 4 PATH PROGRAM DEFINITIONS Imminent Risk of Becoming Homeless – facing eviction without a place to move, living in over-crowded conditions, or in substandard housing, living in temporary or transitional housing, and being discharged from a health care or criminal justice institution without a place to live.

5 5 PATH SERVICES Outreach, screening, and diagnosis Habilitation/rehabilitation Community mental health services Alcohol or drug treatment Staff training Case management Supportive and supervisory services in residential settings Referrals for primary health; job training; educational services Allowable housing services.

6 6 Pennsylvania PATH Program OMHSAS contracts with 19 County MH/MR program offices which comprise 28 counties statewide Some MH/MR provide PATH services directly, while others subcontract to community agencies 30 PATH agencies in the state serving more than 5000 individuals

7 7 PA PATH Program For many counties, PATH grant is the only funding available to serve the SMI homeless population. Innovative programs developed using PATH dollars in conjunction with other funds. New counties/programs funded thru a competitive process if and when SAMHSA increases the yearly grant amount.

11 11 PA PATH Technical Assistance (TA) On June 2nd and 3rd 2008, OMHSAS received a Technical Assistance (TA) award from CMHS. This TA specifically focused on training the PATH providers on innovative and effective Outreach Techniques.

13 13 PA PATH TA - cont’d Relational outreach and engagement model Person-centered, trauma-informed, recovery-oriented care Engagement strategies in rural settings

14 14 NEXT STEPS Continue working with providers to increase PATH enrollment. Continue yearly monitoring to ensure program adherence to established guidelines. Work with individuals from SAMHSA around technical support in outreach.
