2 Wellness Matters This presentation is going to address some important changes that you need to be aware of with regard to – What is changing? – Why is changing? – How will I be impacted? – What do I need to do? – Where can I go for more information?

3 Wellness Matters What and Why? Nothing is more important than our safety and health, wellness is a key component. TimBar offers a very comprehensive Medical (PPO or HDPH), Dental and Vision Coverage to Employees and Dependents and wants to continue to do so. We need to focus on what we all can control Medical care costs are significant concern for us all. It is important for you to take an active role in your own health.

4 Wellness Going Forward TimBar Wellness – A plan to help control costs – Optional and Confidential – Want you to take advantage of free services A physical constitutes participation in TimBar’s Wellness Program and will make you eligible for a lower medical contribution rate if you participate than if you do not. Rates will be set in late fall of each year.

6 Wellness Matters What and Why? You have options for 2013 that will impact 2014  Get a Physical  Do not get a Physical A Physical is defined by TimBar as an appointment with a Physician where you review the following test results: Lipid Profile Blood Sugar Test (Glucose) Cholesterol Screen Blood Pressure

7 Wellness Matters How will it impact me? With a Physical in 2013, you would eligible for a reduction/discount on your bi-weekly payroll Medical contribution starting in January 2014. If your Spouse is also on the TimBar Medical Plan, they will be required to have a Physical for the reduction/discount. In other words, if both you and your spouse are on TimBar’s medical plan both you and your spouse must get a physical to get the discount. Children are NOT required to have a Physical for this reduction/discount.

8 Wellness Matters How will it impact me? The choice is yours: – If you and your Spouse (if applicable) do NOT have a Physical in 2013, then you will pay the highest amount in payroll contributions for Medical coverage in 2014. – The premiums for PPO and HDHP are both impacted. – Rates may vary as much as 30%. (Rates will not be determined until Open Enrollment).

9 Wellness Matters What do I need to do? Next Steps: Schedule your 2013 Physical if you would like to pay the lower rate for Medical contributions in 2014. Get the Physical Verification Form & have your Physician sign the form that confirms you have had your annual Physical at your appointment. If you have already had a Physical in 2013, please provide a copy of your EOB, copy of the services rendered or ask your physician to sign a Physical Verification Form. Return the completed Verification Form to your HR Staff at your Division ASAP, but before 12/31/2013. Once the proper paperwork has been turned in, the discounted contribution will be effective as of January 2014 for the Medical premiums.

10 Wellness Matters Please note, it is your sole responsibility to turn in the Verification Form to your Human Resources Representative (HR). There will be no backdating of contribution levels if you turn in your Physical Verification Form after 12/31 of the calendar year. Your Physical needs to be completed annually within the calendar year to impact next year. Your 2014 Physical will be needed for the reduction on your bi- weekly contribution for 2015. – EXAMPLE A Physical on May 1, 2013. Verification Form Turned in to HR Staff on May 15, 2013. Lower Contributions for Medical begin in January 2014. Next Physical due in 2014 to qualify for 2015 reduced rates.

More information can be obtained from: – Your local HR Rep – Health Advocate – Highmark BCBS (your booklet clarifies all preventative services that are free) – Links on the internet and intranet to important sites and contact information. – Home mailing that will be arriving within the next few weeks.

12 Wellness Matters Keep in mind that personal Confidential Medical Information is not shared with TimBar Packaging and Display. Keep track of your “numbers”, more incentives will follow from time to time. Questions?
