2 What we know: they work best for those with higher levels of need people with higher levels of need are more likely to need both health and social care support They are applicable to mental and physical health They reduce unplanned care They are not right for all NHS Services 2 What the Mandate means: Who benefits?

3 Clarifying the Mandate 3 Still early days, but we are currently exploring : Setting a clear longer term expectation, that in Z years CCGs will spend X% of their budget via personal health budgets or Y% of population. Eligibility numbers would be much greater than actual uptake. Eligibility or budgets could be linked to: top 2% at risk of hospital admissions in line with GP DES, Beyond 2% as part of personalised care planning and supporting people to self manage capitated budgets for people with multiple LTCs Better care fund other areas based on local need and priority Extending the “right to ask” to those who are assessed for NHS Continuing Healthcare but who are not deemed eligible but where they have appropriate health needs Introducing a “right to ask” in mental health for people getting supported in the community NB: These groups are not mutually exclusive.
