3 Accident, Injury, and Exposure Reporting  Policy 100-22  The policy of the Anchorage Fire Department is to:  report all accidents, injuries, and exposures immediately  provide assistance and follow-up for injured or exposed employees  investigate the causes of accidents, injuries, and exposures  reduce the frequency of accidents, injuries, and exposures  maintain complete and accurate records of all employee accidents, injuries and exposures

4 Definitions An accident is any incident involving collision, contact, damage, or loss involving Municipal property (apparatus, equipment, or facilities) or private property. An occupational injury/illness (on-the-job) is any cut, abrasion, bruise, burn, physical pain, illness, impairment of the function of a bodily member, organ, or mental faculty; or any other harm to the body, no matter how temporary, which occurs during, or results from, the performance of one’s duties. An occupational infectious disease exposure (on-the-job) is any specific contact with blood or other potentially infectious materials to eye, mouth, other mucous membranes, non-intact skin, or parenteral penetration of mucous membranes or the skin barrier through such events as needle sticks, human bites, cuts, and abrasions, which occurs during, or results from, the performance of one’s duties. An occupational hazardous materials exposure (on-the-job) is any contact via inhalation, ingestion, eye, skin, or other means with hazardous or toxic materials or chemicals, excessive smoke, or ionizing radiation, which occurs during, or results from, the performance of one’s duties.

5 Accidents  Immediately report all accidents to Dispatch by radio or telephone.  Check for Injuries & provide assistance  Do not move vehicles  Damage exceeds $1000 & involves POV or Private Property  Citation issued or MOA vehicle towed  Post accident Drug testing

6 Occupational Injury/Illness Reporting  Immediately report all occupational injuries and illnesses to Dispatch by radio or telephone.

7 Non-Occupational Injury/Illness (Off-the-job) Reporting  Any employee who becomes sick and/or injured off-duty and is unable to report for work shall, as soon as possible, inform an on- duty officer at their assigned station or work place. For further information, refer to P&I 100-12: Leave.

8 Occupational Exposure Reporting  Immediately report all infectious disease and hazardous materials exposures to Dispatch by radio or telephone.
