5 © 2011 Lantana Consulting Group, 5 Discuss (haven't really worked on this – was just lifted from LCC Use Case): This template represents patient Goals. A goal is a defined outcome or condition to be achieved in the process of patient care. Goals include patient-defined goals (e.g., prioritization of health concerns, interventions, longevity, function, comfort) and clinician-specific goals to achieve desired and agreed upon outcomes. David Tao's comment: These are confusing. Since there is a Health Concerns section that includes prioritization, and an Interventions section that includes interventions, why are they listed here as patient-defined goals? Perhaps it should say alleviation of health concerns, positive outcomes from interventions,… Goals Section Description

6 © 2011 Lantana Consulting Group, 6 Discuss (haven't really worked on this – was just lifted from LCC Use Case): This template represents status, at one or more points in time, of health status evaluations or outcomes related to established care plan goals and/or interventions. David Tao's comment: Added these words because it is not really the status of the goal (e.g., is the goal proposed or actual) but is the status of the evaluations or outcomes for the goal.
