Scoop Tram Roll Over Incidents

2 On September 19 th, an operator was mucking the right hand corner of a pillar in 5156 charging station with a EJC 2 yard scoop.

3 While mucking the bucket caught something solid, the operator lifted his bucket causing the rear of the scoop to lift off the ground and swing to the right, which is down ramp.

4 This caused the left side of the scoop tram to raise approximately 2.5 feet off the ground. The bucket jammed against the face stopping the scoop from completely rolling over.

5 The operator was mucking across a down ramp

 Substandard Action Scoop tram should have come at the pillar at a better angle 7

 Substandard Condition Scoop tram exposed to downhill conditions in two directions while mucking 8

 Job Factors Re-muck should be flat for the first couple of rounds before declining 9

Actions Taken to Prevent Similar Occurrences  Mucking down ramp procedure to be reviewed at line up huddles for every shift  Engineering is looking into starting the re-mucks flatter 10

11 SECOND OCCURENCE

On September 27 th, a worker was mucking a face and re- muck with a EJC 11/4 yard scoop tram. He misjudged where the edge of the muck pile was in the re-muck and ran the left wheel up on the muck pile approximately 1.5 feet. 12

Thus placing the scoop tram off balance. Since the full bucket was raised the centre of gravity caused the scoop tram to flip onto its side. 13

The operator was ejected from the seat and landed on the muck pile. 14

He walked down ramp on his own and was transported to the shaft with a loci. 15

The mechanic was called to secure the scoop, eliminate any fire hazards and rope off the area. 16

The operator sustained injuries to his lower back, left knee and right elbow. 17

 Substandard Actions  Loaded bucket in the air in a hurry  Bucket in the air, left wheel elevated 22

 Substandard Condition  Muck pile encroaching on travel way 23

 Personal Factors  To much in a hurry  Job Factors  Scoop tram not designed to be on an angle with a full bucket in the air 24

Actions taken to prevent similar occurrences  All operators at line up huddles to be reminded of proper scoop tram operation  This will be the topic of the monthly “Safety Theme” 25
