9. 1 question Are you currently receiving or have you in the past received services from Iowa Vocational Rehabilitation or Iowa Dept for the Blind? If yes, your Ticket is being used by them and we want you to keep your Ticket with them.

11. Sign on the Line Together we need to review your information and then both sign. You will get a letter from Social Security stating your Ticket has been assigned.
