6 Survey Dates for Site Coordinators Demographics and Outcomes Survey Open date – start of Phase I Due date – beginning of new nurse enrollment (July 1, 2011), at 12 months (July 1, 2012), and at 18 months (December 31, 2012) End date – 2 months past due date

7 Survey Dates for Study Participants Open date – date when survey is made available to user (7 days before due date) Due date – date when survey is due for completion End date – grace period for survey completion; survey is not available after this date (21 days past due date)

9 Survey Access Each user has individual password- protected access No user can view previously submitted surveys Unable to view individual-level data for other participants Study preceptors, site/state coordinators can view progress of survey completion for each user assigned to them
