CURRENT DELIVERABLES Support and Education Program (AS York) 1,500 families served by social work team 250 of these families utilize the partner ADPs (200 Region of York + 50 York Central Hospital) 15, 000 ADP partner ADP spaces filled 36 client team rounds attended 20 support group sessions facilitated
