2 CCA 2013 Contest Checklist  Remember to include an individual’s name under Entrant Information  Print two copies of the entry form  Attach (paper clip or staple) entry form to manila folder or binder  Place one entry form inside the folder with the entry  Photography entries can be placed in a single manila folder with corresponding entry forms  Email a PDF or JPEG with the Title of Entry_Category#

3 PAYMENT FORMS  Complete a Payment Form for your entries  Print two (2) copies of the payment form  Include one (1) payment form with your entries  Retain one payment form for your records  Mail entries with Payment Form to: CCA Contest c/o Kelly Dolan 2205 Jan Drive Clarksville, TN 37043

4 MAILING PAYMENT SEPARATE FROM ENTRIES?  If mailing your payment after you submit your entries, you can send the payment to the CCA Business office by March 8, 2013  One check can be submitted for an organization. Be sure to include the payment forms for each entrant, along with your check.  If separate from entries, mail your payment to the CCA Business Office: CCA Business Office 174 Crestview Dr. Bellefonte, PA 16823

5 CHART TOPPERS & FLOPPERS This next section features Do’s (Toppers) & Don’ts (Floppers) Hum along, if you’d like.

6 Chart Flopper—Shot Through the Mail Shot through the mail, and you’re to blame. Darlin’, you give Duct Tape, a bad name. Avoid death by duct tape. You can secure your entries with paper clips, binder clips, etc. without going to the extreme.

7 Chart Flopper—Unstapled Melody Oh, my love, my staples have hungered for your pluck. Remove them next time. Your entry forms may not stay in tact once all the staples are removed. A binder clip or paper clip is the best way to attach an entry form to the actual entry.

8 Chart Flopper—Put a Name On It Cause if you like it, then you shoulda put your name on it. If you like it, then shoulda put your name on it. Wha-oh-oh-oh-oh-ooh- oh-oh-oh-oh-oh-oh Be sure to include your name on the payment form, even if your Accounting department will issue payment or you’re using one form to pay for the organization.

9 Chart Flopper—My Favorite Things Raindrops on roses and whiskers on kittens Bright copper kettles and warm woolen mittens Entrants who remember to list their name. These are a few of my favorite things. For Entrant’s Name, be sure to include your name even if you’ll request a department name for the trophy inscription.

10 Chart Topper—Pay Me, Maybe Hey, I just got your form, and this is crazy. I need each entrant, to charge you baby. You can make one payment for the organization, but must send in each entrant’s payment form with your payment.

11 Chart Topper—That’s the Way, We Like It Oh, that's the way, uh-huh uh-huh, I like it, uh-huh, uh-huh. That's the way, uh-huh uh- huh, I like it, uh-huh, uh-huh. That's the way, uh-huh uh- huh, I like it, uh-huh, uh-huh. That's the way, uh-huh uh- huh, I like it, uh-huh, uh-huh. This is an example of an appropriately packaged entry, with the payment form.

12 Chart Topper—Wait, Mr. Postman This year, email PDFs or JPEGs of your entries to the Category emails. Please don’t email your entry forms, only your entries. The PDF files will be used for the Winners’ display in the Contest Room at the Institute. Name your PDF/JPEG files: Title of Entry__Category#  Only email your actual entry, not the entry forms. Entry forms should be shipped with your mailed submission.  Submitting entry forms without the entry doesn’t qualify for the Early Bird deadline.  Be sure to allow enough time for your entry to ship if you’re not using an overnight service. Wait!! Oh yes, wait a minute Mr. postman Wait!! waaaiiit Mr. postman... Please Mister Postman, let me check and see Oh, yeah If I emailed PDFs of my entries

13 Chart Topper—Working 8 to 5 Tumble outta bed And stumble to the kitchen Pour yourself a cup of ambition Yawnin, stretchin, try to come to life Pulling those files And the blood starts pumpin Stack up your entries Your heart starts to jumpin For folks like you on the job from 8 to 5 Don’t let your 8 to 5 get in the way of you entering the CCA 2013 Contest. Get your work the recognition it deserves. Entries must be received by the deadline. Early Bird Entry Deadline—February 15 Regular Deadline—February 22
