1 Electronic Patient Discharge Plan (ePDP) Brief Tutorial For Providers Last updated: 2/9/151

2 What? Electronic Patient Discharge Plan (ePDP) is required for patients discharged or transferred from SFGH to another facility (including inpatient psychiatry, 4A) If you include the Discharge Summary no additional note will be required after this is complete. Inter-service transfer summaries (e.g. MICU to medicine, medicine to surgery) should continue to be dictated or entered directly in LCR Reports/Notes section. Last updated: 2/9/152

3 Why? The new version is meant to provide improved continuity of care between inpatient and outpatient care, improve the safety and quality of discharges at SFGH, and more clearly communicate the hospital course with other physicians. It is required to improve compliance with ACA and Joint Commission guidelines. Last updated: 2/9/153

4 Who? All patients discharged from a med/surg unit. This includes patients who are occasionally discharged from 5E/5R/4E and all patients discharged from 4B step-down and 5D telemetry. Med/surg and pediatric patients discharged from 6A. Outpatient surgery/PACU, ER, L&D, 6C/6H will not participate in this pathway. Last updated: 2/9/154

5 Where? LCR will have a new tab on the left column of the patient’s record titled Discharge Process. Under this tab, select the ePDP option to complete the PDP for the patient. Last updated: 2/9/155

6 When? ePDP can be started at any time during the patient’s hospital course (you are encouraged to start early!) and can be modified by any provider. Last updated: 2/9/156

7 How? The primary provider will complete the required sections and any other applicable sections. You may work on the ePDP at any point during the hospitalization, using the Save button to save work as you go. When the provider is finished with PDP, he/she will use the MD Finalize button to submit and electronically sign the document. The ePDP and discharge instructions will ultimately be scanned and uploaded to the LCR in Reports/Notes. Last updated: 2/9/157

10 Last updated: 2/9/1510 These 3 tabs will serve as the discharge summary. If completed here no additional summary needs to be written!

11 Last updated: 2/9/1511 Click this box if you completed (or will complete) a separate discharge summary instead.

12 Last updated: 2/9/1512 No further information can be added to these tabs once the option is marked as these are the discharge summary fields.

13 Last updated: 2/9/1513 Clearer communication with outpatient providers is one of the benefits of the ePDP. If PCP is within DPH/UCSF system, an email will be sent to the PCP advising him/her to review discharge documents.

14 Last updated: 2/9/1514 Click the SAVE button located at the bottom of each tab at any point while editing the ePDP to save your work. You can then continue working on the document or return at a later time.

15 Last updated: 2/9/1515 After clicking the Save button, you will be required to enter your password to save your work. You or any other provider may then continue working on the saved version at any point in time.

16 Last updated: 2/9/1516 You can edit any section of the ePDP by clicking on the field and then hitting modify below or go directly to the relevant tab above.

17 Last updated: 2/9/1517 To FINALIZE the ePDP go to the Summary tab and click Finalize below.

18 Last updated: 2/9/1518 IMPORTANT: Your signature on the FINAL ePDP indicates that you are accountable for the entire content (even if multiple providers contributed to the document over time).

19 Last updated: 2/9/1519 To make edits to an already finalized ePDP, go to Enter/Update PDP. Click Continue to make changes to the document.

20 Last updated: 2/9/1520 The finalized ePDP is viewable from the Reports/Notes section as “ePDP & Discharge Summary.” Future revisions to the ePDP will update the corresponding document.
