2 COORDINATED PLAN PROCESS Measurable outcomes of the program will be in alignment with one (or more) of the following overarching performance objectives for the National Center for Chronic Disease Prevention and Health Promotion including: Chronic Disease Prevention: –Reduce age adjusted mortality due to chronic diseases; –Reduce prevalence of disabling chronic diseases; Health Promotion: –Improve quality of life and health outcomes by promoting environmental and policy changes pertaining to nutrition, physical activity, clinical preventive services related to chronic disease prevention, early detection and management –Promote education and management skills for those diagnosed with or at risk for chronic diseases.. 3 COORDINATED PLAN PROCESS  Provide leadership for chronic disease prevention and control,  Establish cross cutting policy, communications, epidemiology, surveillance and evaluation activities to support an array of categorical chronic disease programs,  Foster collaboration and efficient use of resources across existing or new categorical programs addressing chronic diseases and their associated risk factors,  Best position programs and resources to achieve population level change in proposed chronic disease and risk factor outcomes,  Identify specific population subgroups that suffer disproportionately from the conditions or risk factors being addressed,  Narrow the gaps in health status between these population subgroups and the population as a whole.  Work at the state and community levels, including in schools, worksites, child and adult care programs, and collaborate with transportation, agriculture and other sectors, and in the health care setting.. 4 COORDINATED PLAN PROCESS The Chronic Disease State Plan should: – describe overall expected achievements for the coming years that will reduce the burden of chronic disease in the state as a whole. –should include strategies that will be led by governmental and non-governmental partners –should not be limited to a plan only for the state health department. »CDC guidance document Cabinet for Health and Family Services

5 COORDINATED PLAN PROCESS Steering Committee October 2011 –Develop and agree on four key strategic areas November 2011-engage a wider group of stakeholders Continue to engage steering committee and stakeholders to review and revise strategies, objectives and tools – (soft target submission August 2012 and completion Spring 2013) Continue to look at adding strategic partners and combined leadership opportunities Communication planning for coordinated messaging Evaluation. 7 COORDINATED PLAN PROCESS Strategic Area 1: Promote policy, environmental and system changes that will support healthy choices and healthy living in Kentucky and its communities. Key Initiatives: Tobacco prevention and control policies Access to healthy foods, and nutrition education Comprehensive physical activity policies and environments in schools, child care centers and communities Worksite wellness policies and programs in Kentucky businesses Breastfeeding strategies and policies. 8 COORDINATED PLAN PROCESS Strategic Area 2: Expand access to coordinated, quality, evidence-based clinical screenings, clinical management and chronic disease self-management. Key Initiatives Evidence-based clinical screenings for chronic diseases including cancer, diabetes, BMI, cholesterol, BP, oral health and depression. Evidence-based clinical management practices and support for chronic diseases including ABCs, tobacco cessation, electronic health record use, medication reconciliation and medication management Provider and individual awareness of and referral to self-management opportunities in the community Reduction in out-of-pocket cost to the consumer for clinical preventive services Patient navigation and coordination of care Alignment with national recognition/certification program standards Health system and quality improvement changes such as electronic health records, systems to prompt clinicians and deliver feedback on performance, and requirements for reporting on outcomes such as control of high blood pressure and the proportion of the population up-to-date on chronic disease screenings can encourage providers and health plans to focus on preventive services. Effective outreach to consumers and reducing barriers to accessing these services is also key, as coverage alone will not ensure use of preventive services. Cabinet for Health and Family Services

9 COORDINATED PLAN PROCESS Strategic Area 3: Cultivate strong connections linking individuals, community organizations, businesses, schools, the health care system and other partners to improve health outcomes, reduce health care costs, and improve quality of life. Key Initiatives: S ustainable networks to address chronic disease prevention and health promotion efforts at local, regional and state levels Community assessments to address health needs and barriers (Hospital community benefit and public health accreditation) Coordinated evidence-based public health education messages engaging the community in programs, policies and practices Availability and utilization of evidence-based chronic disease self-management programs (for Arthritis, Asthma, Cancer, Diabetes, Depression, Heart Disease) Availability and utilization of chronic disease prevention education programs Cabinet for Health and Family Services

10 COORDINATED PLAN PROCESS Strategic Area 4: Translate surveillance, research and evaluation findings into information that is easily accessible to and useful to the community partners, health advocates and decision makers. Key Initiative: Chronic disease surveillance and evaluation systems are needed to describe the extent and impact of chronic disease in the Commonwealth Reports, policy briefings, fact sheets and publications to support key decision-makers in understanding the scope and impact of chronic diseases and their related risk factors as well as best practices to improve health outcomes The integration of surveillance, evaluation and research data into health- related planning and funding decisions Partnerships between academic public health researchers and public health practitioners to identify best practices in real world settings.
