2 Community Outreach — Starting the Conversation It’s About the Conversation, Palliative Care and Advances in End of Life Communication

3 What We Want Versus What Happens 90% of us believe that it’s important to discuss end of life issues with your loved ones. Only 30% do. 60% of us say that it extremely important that your family is not burdened by tough decisions. 70% of us wish to die at home yet 70% of us die in a Nursing Home or hospital.

4 What Matters To You? Remember, this “Event” is of a most personal nature!

5 What is “The Conversation” The conversation is a right that each and every one of us has, that is, to talk about, perhaps the most personal event in our lives. The Event is the Conclusion of our Lives How we have this conversation and with who, is up to YOU! “It’s Never Too Soon But Often Too Late”
