Use the Assessment Tool in the blue ‘Short Cut’ box on the OHS website  Annual TB testing and Immunizations  UMHS employees in specific job titles  Respiratory Protection Fit Testing  UMHS employees who meet Infection Control criteria

Reporting/Scheduling  New Employees  Department determines if employee needs to process through OHS via Assessment Tool on OHS website  Department completes New Employee Routing Form on OHS website for employees who need assessment at OHS and submits online  New employee calls 764-8021 to schedule new employee health screening.  Work Injuries  Employee or Supervisor: Complete online Work Connections Illness/Injury Report Form, print prior to submitting and have employee bring copy to OHS clinic

Reporting/Scheduling…  Physicals/Testing  Hospital – OHS will contact to schedule annual appointment  Campus – Department - Contact OSEH at 615-2140 to determine if your employee needs to be in any program OSEH will submit request to OHS OHS will schedule employee and contact via email  Specific to Department (e.g. Post-offer physicals ) Department contacts OHS to schedule
