1 Member ID Cards

2 New ID Card, New Number Commercial and Medicare Members
All members will get new ID numbers and new ID cards upon migration. As members migrate to the new platform, a new 12-digit number, or UMI, will be assigned along with a 3-letter prefix assigned to the plan’s subscriber Medicare Supplement members will be assigned a 13-digit UMI, with the last digit being an alpha character Currently, we use a 10-digit unique subscriber ID, or USI. This number has been assigned at the member level, meaning each member on the family contract is assigned his own unique number. Cards will have a new member ID number Currently, IBC uses a 10-digit number that we refer to internally as a unique subscriber ID, or USI, to identify each member. Despite the name “subscriber” in the acronym, the identification number is assigned at the member level, which means that each member on the policy is assigned his or her own unique number. As commercial and Medicare members migrate to the new platform, a new 12-digit number, called a unique member ID, or UMI, with a new 3-letter prefix will be assigned to the plan’s subscriber. Medicare Supplement members will be assigned a 13-digit number, with the last digit being an alpha character. All members on that plan will share the same number, whether a subscriber, spouse, domestic partner, or dependent. This number is called the Subscriber UMI. Each member’s card will include the Subscriber UMI, along with his or her name. Removal of Highmark Blue Shield from PPO Cards On cards issued to PPO members, the line “Certain Services Provided by Highmark Blue Shield” will be removed. On or after November 1, claims for covered services from Highmark professional providers inside IBC’s five-county service area (Philadelphia, Bucks, Chester, Montgomery, and Delaware counties) will continue to be processed as out-of-network claims, but PPO members may be balance billed by the provider. Claims for covered services from Highmark professional providers outside IBC’s five-county service area will be treated in the same way they are today. 2

7 Using the New Card New cards will be issued to members by mail prior to the group’s effective date on the new platform. Instructions on how to use the new card will be provided with the card. If a member uses the card prior to the group’s effective date on the new system, a process is in place to map the new and prior numbers so claims process correctly. Network providers and pharmacies have been notified of changes in the ID numbers and reissuance of cards. IBC will issue new ID cards to members at a customer’s migration. Please note that all members will receive a new ID card at migration, regardless of whether or not the group made changes to benefits. We will also include instructions to explain the changes to the new card and how to use it. If a member uses the card prior to migration, there are safeguards in place to map the new and prior ID numbers so that claims will process correctly. Likewise, if a member uses his or her old card and ID number, a systematic crosswalk will be in place to ensure proper processing. In addition, our network providers and pharmacies are being notified of the changes to the ID numbers and the reissue of new cards.
