1 Federal Aviation Administration Summary of Recent Safety Recommendations from Aircraft Accident Investigation Authorities Sources: Korean Aircraft and Railway Accident Investigation Board (KARAIB). Asiana 747 Inflight cargo fire near Jeju, Korea. July 28, 2011 United Arab Emirates, General Civil Aviation Authority (GCAA). UPS 747 Inflight cargo fire. Dubai, UAE. Sept. 3, 2010 US National Transportation Safety Board (NTSB). UPS Dubai accident and UPS DC-8 inflight cargo fire. Phila, PA. Feb. 7, 2006 1

2 Federal Aviation Administration 2 Require the installation of fire suppression systems in all cargo compartments on commercial aircraft. Require fire detection systems that provide early warning to the flight crew for fires originating inside cargo containers or pallets wrapped with protective covers. Reduce the hazards from air transport of lithium batteries and other hazardous materials through improved packaging, quantity limits, location, proximity to other hazmat, detection of undeclared hazmat, training, inspection, supervision, and crew notification Require systems that provide information to the flight crew on the location and severity of inflight cargo fires.

3 Federal Aviation Administration 3 Improve aircraft ability to prevent smoke penetration into the flight deck. Require increased protection of aircraft systems critical to continued safe flight and landing from the effects of a cargo fire. Develop standards and require the use of fire resistant cargo containers (ULDs) with built in fire suppression systems. Improve checklists to better enable flight crews to respond to inflight cargo fires. Require the implementation of vision assurance devices or technology to allow flight crews to see and operate aircraft controls in a smoke filled flight deck.
