2 Why did you join the DPP? I wanted to help find a cure!
It wasn’t a contract. I could quit. I found out I was at risk and said… Okay I need to do this for my family. I wanted to be in the lifestyle group. I didn’t want to take medicine. I wanted to be in the lifestyle arm but would have stayed in if I had been put on medication. Certainly everyone wanted to delay or prevent the onset of diabetes, they wanted to contribute to their community as well as a sense that the were contributing to the greater Indian Community.

3 TO CONTRIBUTE TO THEIR COMMUNITY AND TO THE GREATER INDIAN COMMUNITY

4 What Did You Do? Exercised! I didn’t think I could!
I started with food on a scale. It showed me portion sizes. Logging in what I ate and keeping track of activity. Having accountability, I had to answer to someone. Many were concerned about being able to do the exercise but were pleasantly surprised. Food records and measuring (DPP provided the tools) ACCOUNTABILITY ACCOUNTABILITY CASE MANAGERS, PERSONLA & GROUP.

5 What were the highlights?
Meeting different people (Met tons of people!) Cooking class The group The cups, T-shirts, bookmarks, magnets, postcards, shoelaces, reminders Still write letters to one of the Salt River Sisters! I FELT BETTER the first week SOCIAL ASPECT-Getting together HIDDEN HIGHLIGHT EXCUSE TO PARTICIPATE You can’t be too different or people might make fun- Group challenges THERE WAS ALMOST A SENSE THAT

It became very clear that the case managers techniques were a critical factor. Something else was clearly going on with these participants. There was something almost magical. There lives were better. HOW MANY OF YOU HAVE ALREADY GONE TO THE WEBSITE AND DOWNLOADED MATERIALS?

8 What was the KEY to your success?
“This may not be related to the program but just being able to talk to someone. They were a sounding board.” “Being able to talk to someone who cared. They NEVER gave up on me.” “No matter what, I never heard the word fail.” “My coach always tried to find ways to motivate me. Exercise videos, recipes, walking tapes, but she was NEVER PUSHY.”

The five steps to solving a problem: Describe the problem in detail. Brainstorm your options. Pick one option to try. Make a positive action plan. Try it. See how it goes.

HOW MANY OF YOU EITHER HEARD OR SAID TO YOURSELF Amazed how these participants who clearly had lots of “life issues” worked on their own plans for making lifestyle changes that fit into what they were able to do - INDEPENDENT of those life issues!! Everyone

13 Retention After 10 years for some: 30% regular attendance
98%+ attending data visits 70% below their baseline weight (Lifestyle) 2005 “Amnesty Class” 100%!! Still Connected to those who don’t regularly attend class to get them in when we need to-No broken arms needed. Many participants attend when they can.
