2 www.monash.edu.au 2 Care leavers are vulnerable and disadvantaged group; Evidence of poor mental and physical health, and causes of problems; Need regular health assessments, and access to therapeutic counselling.

3 www.monash.edu.au 3 Also evidence of substance abuse and causes of problems; Out of home care programs can assist with rehabilitation.

4 www.monash.edu.au 4 St Lukes Anglicare leaving care and after care support service based on strengths-based approach; This paper is part of larger study of effectiveness of St Lukes model; Qualitative interviews with 19 young people, and follow up with 11 young people.

5 www.monash.edu.au 5 At first interview health varied. Some significant physical health problems, and large number had anxiety or depression. Also unresolved anger. Six of 19 had significant substance use concerns.

6 www.monash.edu.au 6 At second interview, improvements in mental health for 11 young people. Also reduced substance use. Associated improvements in housing and education/employment.

7 www.monash.edu.au 7 Some limitations of study. But confirms disproportionate health concerns. Deficits in areas of disability support, access to therapeutic care, and serious substance use. Confirms importance of detailed leaving care plans addressing health issues, and ongoing holistic post-care support.
