8 Leveraging Community Resources The Access Network has an unmet needs fund called the Flexible Funding Pool of between $60,000 and $100,000 (capital) each year. This allows community resources to be leveraged 2:1 or 3:1.

9 Results 2-1-1 in 2014: 940,000 calls 91,000 from seniors Projects up to 180,000 by 2020

10 Results Since July 2009… 434,000 calls from older adults Senior Specialist in-depth assessment with 8,231 clients In-home, long-term Case Management with 1,258 clients 12 Provider Partners 20+ Community Resource Partners

11 Lessons Learned 1.Who is already invested in doing the work? Start there. 2.Create the vision together AND standards of care – hold each other accountable. 3.Everyone brings resources to the table (mostly sweat equity). 4.Money gets it started, but money alone won’t keep it going. 5.Add to the work in a way that is responsive to stakeholders and the community. 6.Ongoing meetings for training, support, and just being together. 7.Two most important budget items: unmet needs fund (for clients) and food budget (for meetings).
