www.mscvd.org Overview Mississippi has the dubious honor of being Number 1 in heart disease morbidity and mortality IN THE NATION Legislature created Governor’s Task Force on Heart Disease and Stroke Prevention to create solutions –CVD State Plan is one result

www.mscvd.org Mississippi Has a CVD Problem! Three-fourths of Mississippians have at least one CVD risk factor Racial / ethnic / geographic disparities are central issues Primary prevention is a KEY

www.mscvd.org Pointing the way to a new future BOLD new plan for reducing CVD in Mississippi

www.mscvd.org Improving Mississippi’s CVD Outcomes Enhance collaborations and partnerships among key constituencies at all levels; Implement program services that encourage healthy communities; Increase public awareness of CVD risk factors, disease management, and early recognition and response to acute events; and Impact policies that build healthy lifestyles and communities, and promote wider access to health care.

www.mscvd.org Focus of Plan: CVD Health & Health Disparities* Identify and reduce health disparities in CVD in target communities Priority populations: Low SES, rural, women and African Americans * Age, gender, ethnicity, economics, education, community resources, risk factors, and barriers

www.mscvd.org Interventions targeted toward: Prevention and management of CVD risk factors –nutrition, physical inactivity, tobacco, sociocultural Management of risky conditions –hypertension, obesity, dyslipidemia, diabetes Management of acute events –Heart attack, stroke

www.mscvd.org Implementation Strategy Invest in school, faith-based, worksite, civic, health care providers to leverage actions toward priority populations Address community-specific issues

www.mscvd.org Core Strategies Disseminate the Plan to all key constituencies Develop state –wide CVD risk factor registry “Know Your Numbers” as centerpiece activity across all venues Develop and disseminate “tool kit” for use in all venues
