12. What happens next? Fill out an application form Return the form and £150 deposit by 24 th April 2015 First come first served basis in each team Top 8 get priority, then reserve list will be formulated If somebody drops out, the person who is number 9 on the list will be contacted This person will be expected to pay the balance to that point Therefore, we advise that number 1 and 2 on the reserve list account for this

13. Payments 22 months payments £81 a month Standing order must be set up for the 4 th of each month First payment will need to be made on 4 th May 2015 The final payment will be made on 4 th March 2017 Therefore the total cost will be £1932 Sixth form Follow the payment plan If you choose to attend a different sixth form, you will be refunded in full (including the deposit)

Fundraising We will need to raise £3200 for kit Therefore, when pupils pay the deposit, they must also sign the bottom of the form to confirm that they agree to be involved in all fundraising activities If pupils have a genuine reason to miss fundraising activities, they must be excused by a member of staff.
