7 What is Patient- and Family-Centered care?
“Providing care that is respectful of and responsive to individual patient preferences, needs, and values and ensuring that patient values guide all clinical decisions.” The Institute of Medicine “An approach to the planning, delivery, and evaluation of health care that is grounded in mutually beneficial partnerships among health care providers, patients, and families.” The Picker Institute ED - 1

8 Core concepts of patient- and Family-centered care
Dignity & respect Build patient/family knowledge, beliefs, culture into plan of care. Listen to, honor choices. Information sharing Share complete, unbiased, timely info. Know patients’ health literacy & act accordingly. Participation Support patient involvement at the level they choose. This level may change over time. 12 2015 Statement of Work The Network shall develop, facilitate and continue a sustainable Patient Engagement Learning and Action Network in the Network area to promote patient and family engagement, patient- and family-centered care, and protect the interest of patients. The LAN shall be patient-driven with topics chosen by the Patient Subject Matter Experts (Patient SMEs) participating in the LAN. OC

13 What is a Learning and action Network (LAN)?
An organized effort of sharing learning by a network of organizations or teams to: Adapt known BEST PRACTICES for a specific health priority or issue, Achieve significant results in a short period of time, Scale up BEST PRACTICES to other organizations via a planned SPREAD strategy.
