3 Recommendations for future meetings Administration Details : Meetings held quarterly, with every second meeting at a RACF Attendees from RACFs HML, ASET Nurses, ASNSW, and HNE Agenda items, minutes, and copies of presentations to be provided to all attendees after the meeting Identify a group to action outcomes of meetings Meeting held in conjunction with the RACF Collaborative Meeting

4 Recommendations for future meetings Case studies /Experiences/ Feedback The agenda to include case study from all partners (RACF’s/ASET / Ambulance) Discuss individual experiences, both positive and negative Pick a patient to follow though the entire process and identify any areas of improvement. discuss how systems/ processes are working and identify areas of improvement Develop strategies for issue resolution

5 Recommendations for future meetings Improve communications: Information sharing and problem solving between ASET and all agencies Close the gap between agencies in real time, do not wait for quarterly meetings for feedback to occur Establish an escalation process or feedback loop to resolve handover concerns. RACFs need to be informed if there is a problem with a handover in order to educate their staff. Develop a protocol on how to have difficult conversations with other facilities about problems, issues or concerns.

6 Recommendations for future meetings Build Interagency Relationships: Visit key stakeholder facilities Understanding the roles of other facilities – spend time with other facilities to see how the other side operates for a day GP liaison and engagement

7 Recommendations for future meetings Resources/ education and technology: -Review and update the manual -Ongoing training and education for staff -Provide updates on what is new in Health -Identify areas where Telehealth may be useful
