18 Potential barriers Competing priorities Lack of energy Lack of vision Too busy with WL initiatives It is the responsibility of the ‘IT Department’ “Just tell us when you have done it!”

19 But we are privileged, and I believe we have a duty We can truly make a difference in the NHS We can see some of what needs to be done We have skills and experience to make some of it happen Integrated care needs integrated information

We will be part of a flourishing network of health and social care partners to integrate care for our patients, deliver clinically excellent results and be financially sustainable. Create networks for acute and complex care. Develop integrated care for people with long-term conditions to help them to stay as healthy as they can be.”

30 How do we do that then? Above all we should hold on to the thought that if we want to get the same results, we should do what we have always done. If we want a different outcome we should try different things.

31 We need an integrated NHS. Patients need integrated information. My dad needs both. This is my challenge to all of you.
