Key words…  ‘Easier’  You decide your health goals (which need to be measured and monitored with your agreement)  You decide the best way to spend your money (with your local NHS team)  You have more say…

A & M film autumn 2014  ‘Alan’ is only the second ever patient in Trafford to be awarded a PHB  He was already a private customer of Bluebird Care Trafford  Later-stage MND  We helped A & M apply for A’s PHB with the help and guidance on NHS Trafford PHB but we were all breaking new ground together  His first application was turned down due to insufficient ‘outcomes’ evidence. We has to pioneer a new way through  CONSIDERABLE extra work from Bluebird Care Trafford (two weeks unpaid) with A and NHS Trafford CCG staff made the second correct application successful in October 2014  PHB backdated in full from October 2014 to April 2014  Occasional hiccups in March 2015 accessing the finances but relatively smooth from now on  Learned some valuable new lessons as part of the ‘triangle’ that can be put into practice from now on

Our virtuous circle Successful business with an outstanding reputation Strong business growth and revenue to invest Attract the very best care colleagues Give them the very best training, support and £package Fantastic work from an engaged committed and consistent team Very happy customers (who tell other people)  Management Team & development  Business planning & strategy  Financial planning & strategy  Marketing  Recruitment  Induction  Ongoing training  KPI/metrics  Continuity of care focus  Customer and staff satisfaction

Some barriers still to overcome…  ‘Silo’ mentality in the care sector  ‘You’re not as well qualified as we are..’ a.k.a. ‘WTHAY?’  The sector isn’t vertical, it’s really horizontal…  ‘Horizontal’ is proper integration  A crying need for common ‘outcomes’ methodology from the outset (otherwise there’s ‘scope creep’, wasted effort and potential confusion)  The great news is… NHS Trafford CCG is clearly committed to helping patients and providers come together. We can testify to this.  Our experience is that NHS Trafford CCG is clearly committed to working with ‘best of breed’ providers and helping them enhance their services even further, if…  Providers are also committed to helping themselves first by identifying needs-gaps and training-gaps and meeting the CCG halfway on costs and implementation  It’s a ‘win-win’

Our Proposition What we stand for

Whole Person Care TM  We care for the individual  We tailor their care plan so that it is person-centred  As a customer you will receive a unique combination of excellent physical and emotional care  We look after the ‘Whole Person’ – not just your body, or not just your emotional wellbeing, but both, as well as your wider family  We make sure those customers with dementia are properly hydrated and nourished, and help them with social inclusion and exercise  We allow those at the end of their lives to ‘die a good death’, with dignity and respect for the person  We make it our business to learn and know more in depth knowledge about dementia and specific neurological conditions, to give you a better life  And we measure your satisfaction and try to prove the beneficial impact of our care on you, so that you can live longer and better than you would without us (true added value)

Evidence (reasons to believe)  Excellent customer feedback – will move to NPS in 2015  M&H TTT embedded in the organisation – we do this better and in more depth – ‘zero tolerance’ on pressure sores: community nurses praise us for this  Links with community specialists and charities/groups (AgeUK Trafford; NHS Trafford CCG; Pennine NHS Trust; GPs; DNs; SCOPE; MNDA; Alzheimer’s UK etc. etc. etc.  Whole Person Care TM - Customer outcomes measurement tools being investigated and trialed – implement the best in 2015 (ASCOT/FACE/Outcomes Star?)  Innovation & efficiencies – ‘PASS system’ trial – real-time reporting  iConnect / call monitoring enhancements

Integrated staff training programme for neuro-degenerative care (co-production with third parties)  Palliative care / End of Life care specialism and further developments  Motor Neurone Disease  All Dementias / Alzheimer’s Disease  Parkinson’s Disease  Stroke Care  Cerebral Palsy (& other Palsys)  Acquired Brain Injury  Autism Spectrum Disorders (incl. Asperger’s)  Epilepsy  Children’s Care

Key words…  ‘Easier’  You decide your health goals (which need to be measured and monitored with your agreement)  You decide the best way to spend your money (with your local NHS team)  You have more say…  ‘Alan’ has just upgraded his touch/type app again using his PHB so he can still communicate, now he can barely type any more. He still has his say…
