How to Calculate Patient Meal Percentage Intake  Water, tea, coffee, and condiments (ketchup, mustard, and mayonnaise) do not count  Butter counts with the bread  All other food items are given 1 point  All or ¾ of a particular food item=1 point  If the patient eats ½ of a food item=½ point  If the patient eats ≤¼ of a food item=0 points  Count the points eaten  Divide by the total items on the tray After viewing entire presentation, click here to begin quiz.here

Adding Up the Points  Total Items on Tray 1 2 3 4 5 Tip: Use the Tray Ticket to count how many items were served. Example: 5 items on tray (two potato wedges=1 serving)

Doing the Math Of the 5 items on the tray:  ¾ of potato wedges=1  All of chicken=1  All of banana=1  ½ broccoli  ½ milk Do the math 1+1+1+½+½=4 4 / 5 =80%
