4 Current Best Practices Report of Accident (ROA) submitted to L&I in 2 business days Activity Prescription Form (APF) when a worker has restrictions, cannot return to full-duty, or will be claiming time-loss Provider contact with the employer Barriers to recovery assessment

5 Report Of Accident (ROA)  Purpose: –Allows a worker to access medical and lost work benefits –Allows the health care provider to be paid for their services  Focus on timeliness and completeness –ROA to L&I within 2 business days
