2 General Radio Failure Procedures
a) Specific procedures for countries and aerodromes are described in Volume 1 of the documentation in section JEPPESEN "EMERGENCY". b) The following procedure is the ICAO procedure and has been approved by the GCAA published in the Operations Manual of DOT LT Gabon. The SA procedure is published in Part 3 of the OM In VMC: a) Continue to fly in visual meteorological conditions; b) Land at the nearest suitable aerodrome; and c) Report the arrival by the most expeditious means to the appropriate air traffic control unit In IMC or when conditions are such that it does not appear likely that the pilot will complete the flight in accordance with the prescribed VMC procedures above: a) Unless otherwise prescribed on the basis of a regional air navigation agreement, in airspace where procedural separation is being applied, maintain the last assigned speed and level, or minimum flight altitude if higher, for a period of 20 minutes following the aircraft’s failure to report its position over a compulsory reporting point and thereafter adjust level and speed in accordance with the filed flight plan; or . 26/10/2013

3 General b) In airspace where an ATS surveillance system is used in the provision of air traffic control, maintain the last assigned speed and level, or minimum flight altitude if higher, for a period of 7 minutes following: The time the last assigned level or minimum flight altitude is reached; or Time the transponder is set to Code 7600 is set to indicate the loss of air-ground communications; or The aircraft’s failure to report its position over a compulsory reporting point; whichever is later and thereafter adjust level and speed in accordance with the filed flight plan; c) When being vectored or having been directed by ATC to proceed offset using RNAV without a specified limit, proceed in the most direct manner possible to re-join the current flight plan route no later than the next significant point, taking into consideration the applicable minimum flight altitude; d) Proceed according to the current flight plan route to the appropriate designated navigation aid or fix serving the destination aerodrome and, when required to ensure compliance with e), hold over this aid or fix until commencement of descent; e) Commence descent from the navigation aid or fix specified in d) at, or as close as possible to, the expected approach time last received and acknowledged; or, if no expected approach time has been received and acknowledged, at, or as close as possible to, the estimated time of arrival resulting from the current flight plan; f) Complete a normal instrument approach procedure as specified for the designated navigation aid or fix; and g) Land, if possible, within 30 minutes after the estimated time of arrival specified in e) or the last acknowledged expected approach time, whichever is later 26/10/2013

4 Flight Duty period Gap analysis Between DOT and Shell contract:
The general rules which shall be applied to aircrew are as follows:  Rest periods shall be a minimum of 12 hours. A formal schedule for rest periods shall be laid down. This may be reduced to 11 hours where accommodation is close to the place of work; DOT LT The minimum rest which must be provided before undertaking a flight duty period starting at home base shall be at least as long as the preceding duty period or 12 hours whichever is the greater; Conclusion – DOT LT procedure is valid They shall not work more than seven consecutive days between days off and shall normally have not less than 2 consecutive days off in 14; The minimum rest provided as outlined above is increased periodically to a weekly rest period, being a 36-hour period including two local nights, such that there shall never be more than 168 hours between the end of one weekly rest period and the start of the next. Conclusion – in addition to DOT LT Shell procedure shall be used 26/10/2013

5 Flight Duty period Conclusion – not applicable
Dispensation may be given if Crews are rostered to be on site for limited periods interspersed with extended leave periods (e.g. 14 days on, 14 days off) but discretion will need to be tempered by the intensity and nature of the flying operation. SAI advice should be sought; Conclusion – not applicable They shall average at least eight days off in each consecutive five-week period averaged over three such periods; Conclusion – not applicable –DOT rostering 4 weeks on 26/10/2013

6 Flight Duty period  For split duties with less than two hours rest within the FDP no extension shall be allowed. With two to three hours rest, the FDP may be extended by one hour and with 3-10 hours rest, the FDP can be extended to half the consecutive hours rest, which shall not include the time for post and pre-flight duties. With more than six hours consecutive rest, suitable accommodation must be provided;  DOT LT Break on the ground shall be taken into account with 50%.
If a break exceeds 4 hours and the duration of this has been announced in advance, it shall be taken into account with 0%. During a break the crew member shall have access to lying rest in a rest area on ground. Conclusion – in addition to DOT LT Shell procedure shall be used Records for duty and rest periods of all flying staff shall be kept and rostering staff shall make reference to the records before selecting a Crew for a task.; 26/10/2013

7 Conclusion – in addition to DOT LT Shell procedure shall be used
Flight Duty period For Ad Hoc/Call off contracts where full application of these limits is not possible, at a minimum the maximum FDP shall be observed while flying for Shell, and the Pilot shall have 12 hours of uninterrupted rest (reducible as above) prior to reporting for duty on a Shell flight; DOT LT The minimum rest which must be provided before undertaking a flight duty period starting at home base shall be at least as long as the preceding duty period or 12 hours whichever is the greater; Conclusion – DOT LT procedure is valid  The Operator shall have a process to monitor and control Crew member fatigue resulting from travel to work (commuting) from a non-local location while on days off. At a minimum, the Operator shall ensure that Crew member travel while on days off during the 12 hours prior to reporting for duty does not exceed two hours, and that the Crew member has the opportunity for ten hours of actual rest local to the workplace. If necessary the Operator shall require the Crew member to report to the work site no less than 11 hours prior to coming on duty for Shell charter and then be provided local accommodations; Conclusion – in addition to DOT LT Shell procedure shall be used 26/10/2013

8 Flight Duty period Any concern about the FDPs being applied is to be referred to SAI. Conclusion – in addition to DOT LT Shell procedure shall be used  In addition to the SGRAO requirements: Two pilot operations using split or continuous duty periods which total more than 13 hours from beginning to end shall still comply with the minimum rest requirements. Next day start times shall be adjusted as necessary.  For single day ad hoc charters, the crew shall receive the Shell specified rest period prior to reporting for Shell duty. 26/10/2013

9 Flight Duty period While in Company service crews shall be provided:
(i) one hour at the beginning of a crew duty period for preflight preparation, and; (ii) a minimum of ½ hour break near the middle of their duty period for a meal which shall not start until the crew is at their meal, and; (iii) necessary breaks for personal comfort, and; (iii) at least ½ hour at the end of the duty period for post-flight duties. (iv) These times shall be included in the crew daily FDP total. Conclusion – in addition to DOT LT Shell procedure shall be used 26/10/2013

30 Safety induction Afrijet policy on non-punitive hazard reporting
1. Afrijet is committed to the safest flight operating standards possible. To achieve this this, it is imperative that we have uninhibited reporting of all incidents and occurrences which may compromise the safe conduct of our operations. To this end, every employee is responsible for communicating any information that may affect the integrity of flight safety. Such communication must be completely free of any form of reprisal. 2. Afrijet will not take disciplinary action against any employee who discloses an incident or occurrence involving flight safety. This policy shall not apply to information received by the Company from a source other than the employee, or which involves an illegal act, or a deliberate or wilful disregard of promulgated regulations or procedures.Negligence or deliberate violations will not be tolerated by Afrijet. 3. The primary responsibility for flight safety rests with line managers, however, flight safety is everyone’s concern. 4. Our method of collecting, recording and disseminating information obtained from Air Safety Reports has been developed to protect, to the extent permissible by law, the identity of any employee who provides flight safety information. 26/10/2013

31 Safety induction Reporting System of Afrijet:
Afrijet has incorporated an anonymous Flight Safety Reporting System which shall be used by everyone involved in operation irrespective of his or her position. Reports shall be either filed in writing using anonymous report forms available at the operation and Maintenance facilities of Afrijet. 33 Safety induction Airport Operations
All operations within Libreville and Port Gentil Airports are under the control of Afrijet. When in these airports Shell Gabon staff will comply with the Afrijet safety rules and procedures in place. All operations within Gamba airport are conducted by Afrijet but are under the management of Shell Gabon aviation department staff. Shell Gabon DOT LT and Afrijet staff will comply with the Shell Gabon safety rules and procedures in place. All operations within Rabi airport are conducted by Shell Gabon under the management of Shell Gabon aviation department staff. Shell Gabon Afrijet and DOT LT staff will comply with the Shell Gabon safety rules and procedures in place. In all cases, DOT LT aircrew must stop any aviation operation, where there are safety concerns, regardless of who is managing that location. 26/10/2013

34 Safety induction Passenger Check-in Passengers travelling to/from Shell Gabon locations check in for their flight at the Afrijet check-in at Libreville, Port Gentil and Gamba airports and at the Shell Gabon check-in at Rabi airport. During the check in the following details are confirmed:  Flight Booking Flight bookings are checked and only those passengers with authorised bookings will be allowed to check-in for the flight.  Identity The identity of the passenger is checked.  Baggage Weight of baggage to be checked within the prescribed limits. 32kgs per passenger checked baggage 50kg max weight of single freight item* Under exceptional circumstances, items over 50 kgs may be carried following a risk assessment being carried out and documented. Any such items which are easily handled by 2 or more people may be accepted up to a maximum of 90kgs  26/10/2013

35 Safety induction Security
Security staff conduct a security search of all passengers and baggage before flight departure. The purpose of these searches is to prevent prohibited items being carried on Shell Gabon flights. As with other flights, the following items are prohibited: Explosives; knives; compressed gases; flammable substances; substances with a high oxygen content, e.g. peroxides and bleaching-powder; toxic substances; radioactive substances; corrosive substances; magnetic, harmful or malodorous substances and irritants Other illegal or prohibited items. 26/10/2013

36 Safety induction Safety Briefing
DOT LT cabin staff will ensure all passengers are provided with a suitable safety briefing before each take-off. The safety briefing will include:  Aircraft safety features / procedures; Survival equipment briefing / familiarisation;  Personal Protective Equipment Afrijet and DOT LT will ensure that all passenger seats are equipped with a serviceable life jacket. In addition, Afrijet with DOT LT will ensure that all other mandatory safety equipment is installed in the aircraft. Freight Shell Gabon will ensure that all freight to be transported by air is appropriately packaged and correctly booked. Afrijet will ensure that all freight delivered for air transport is correctly packed and manifested. Any freight that is found not adequately packed, or does not have a valid flight booking, will be rejected and held in quarantine. Flight All flying operations are conducted in accordance with DOT LT procedures and Shell Gabon’s Manual of Permitted Operations. All Shell Gabon personnel will adhere to the applicable DOT LT procedures. 26/10/2013

38 Safety induction Adverse Weather
Due to the adverse weather conditions experienced in Gabon at certain times of the year, DOT LT will operate in accordance with their Operations Manual requirements for ‘Flight Procedures in Thunderstorms and when ceiling is below minimum safety altitude.  Jet A-1 Fuel  All aviation fuel used by Afrijet in Libreville and Port Gentil airports will be obtained from an official source and any necessary quality checks will be confirmed by Afrijet staff at the point of delivery into the aircraft.  Aviation fuel used by Afrijet at Gamba airport will be supplied/controlled under the following responsibilities: 26/10/2013
