3 Individual whole life Insurance issued in small face amounts for use in planning of final expenses. Product & Plan Types 100% of the policy’s face amount upon issuance of the policy and includes an Accelerated Death Benefit Endorsement; not applicable in all states. Level Except for an accidental death, the benefit during the first policy year is 30% of the policy’s face amount; during the second policy year, the benefit is 70% of the policy’s face amount and thereafter, the benefit is 100% of the policy’s face amount. The Graded policy is not offered in all states. Graded Except for an accidental death, the benefit during the first two policy years, is a return of premium plus 10% interest. (Level, Graded, Modified) You understand that if you’re face to face with the client then you must be on a two way call. You have read the Medical Authorization Form (HIPAA) to the Proposed Insured. You use the Appropriate language for the way you are submitting: Paper, Eapp or Telesale The Application date is consistent with the Apptical health interview date. The client is a U.S. Citizen. Before you call Apptical, make sure you are prepared: You’ve thoroughly and truthfully asked all health questions listed on the application prior to contacting Apptical. For agent use only.
