1 Conflict Checking for Therapeutic (and Generic) and Duplicates Enabled? YES This is enabled for Provider Order Entry, and a provider comment is required to complete the order entry. Therapeutic Duplicate is defined as 2 drugs that are classified in the same Therapeutic Class. Examples: Nifedipine and Amlodipine Generic Duplicates are defined as 2 drugs that are the same ingredient. Examples: Acetaminophen tablets and Acetaminophen suppositories ***Note*** Generic Duplicates are a sub category of Therapeutic Duplicates, and therefore the Soarian alert verbiage is the same as the Therapeutic Alert (there is no alert verbiage that will display as “Generic Duplicate Alert”). 1

2 *** Note*** The setting “Suppress Duplicate Alerts between New Medication Orders Placed during an Ordering Session” is enabled. This means that if 2 orders for the same ingredient are ordered in the same ordering session, the alert will not display. Example: Percocet 1 tab every 4 hours prn mild pain and Percocet 2 tabs every 4 hours prn mild pain (ordered at the same time). If ordered separately (in separate order sessions), the alert will display. 2

3 Conflict Checking for Allergies Enabled? YES This is enabled for Provider Order Entry, and a provider comment is required to complete the order entry. Conflict Checking for or Adult Max Enabled? YES This is enabled for Provider Order Entry, and a provider comment is not required to complete order entry. Enabling this setting will alert the provider when an order is detected that will exceed the defined recommended Adult Max Dose for that drug. Conflict Checking for or Pediatric Max Enabled? YES This is enabled for Provider Order Entry, and a provider comment is not required to complete order entry. Enabling this setting will alert the provider when an order is detected that will exceed the defined recommended Pediatric Max Dose for that drug. **Note** the alert verbiage is the same for adults and pediatrics. Pediatric Age Cutoff=18 years 3

4 Conflict Checking for or Drug-Drug Interaction Enabled? YES This is enabled for Provider Order Entry for Level 1 interactions only, and a provider comment is required to complete the order entry. Definition of Level 1 Drug Interaction= Contraindicated Drug Combination: This drug combination is contraindicated and generally should not be dispensed or administered to the same patient. Example of Level 1 Drug Interaction: Amiodarone and Quinidine 4

5 Pregnancy Precautions Checking Enabled? YES This is enabled for Provider Order Entry Level 1 interactions only, and a provider comment is required to complete the order entry. Definition of Level 1 Precaution= Contraindicated with Pregnancy. Lactation Precautions Checking Enabled? YES This is enabled for Provider Order Entry Level 1 interactions only, and a provider comment is required to complete the order entry. Definition of Level 1 Precaution= Contraindicated with Nursing Mothers. 5

6 Geriatric Precautions Checking Enabled? YES This is enabled for Provider Order Entry Level 1 precautions only, and a provider comment is required to complete the order entry. Definition of Level 1 Precaution = Drugs with a precaution level of Contraindication (not recommended for geriatric patients). Geriatric Age = 75 Years This is enabled for Provider Order Entry for Level 1 and 2 precautions, and a provider comment is required to complete the order. Precaution level 2: Relative Contraindication (should be evaluated carefully for pediatric patients). Precaution level 1: Absolute Contraindication (not recommended for pediatric patients). Note : The age information in the alerts come from First DataBank and might be more specific that the age cutoff that you set. For example, if you set an age cutoff of 18, provider would receive alerts based on that age setting, but the content of some alerts might specify different ages, as in the following example Pediatric Precautions Checking Enabled?
