49 Contributions in the 3rd year of K4CARE project:  a socio-medical team has been organized to the Day Center for the Diagnosis and Treatment of Memory Impairment established by Ana Aslan International Foundation in Bucharest, in order to perform patient’s data insertion in EHCR  the selection of the out-patients and in-patients (elders with/or motor/psychic disabilities) and their relatives was performed by using their clinical histories after getting their consent  The completion of an appropriate database of the patients to be included in the K4CARE’s EHCR  the socio-medical team and the other actors involved (patients and relatives) have been trained during several local meetings  the socio-medical team has prepared and inserted the data of 60 patients into the EHCR (clinical history, anamnesis, assessment scales: ADL, IADL, MNA, Tinetti, Clock Drawing Test, GDS, Parkinson Disease Rating Scale, diagnostic examination, specialty consultations, laboratory analyses, treatment etc).
