Expand Care Coordination statewide
North of Bangor, Downeast
Focus on recently separating veterans & their families (within 2 years of service separation)
Work shoulder to shoulder with grant partners to help veterans navigate multiple support resources

Care Coordination provided to veterans over the next 2 years
Help avert suicide
Link to appropriate resources for VA benefits, health & mental health care
Refer to DOL, assist in job searches, college enrollment
Aid in preventing homelessness

Lois Jones, LCSW, supervises clinical aspects of the service statewide

Ellen McGuire serves Bangor to Brunswick
Sally Papciak serves Brunswick to Kittery
Currently interviewing candidates for serving points north of Bangor
Stacy Leet, MHRT/C, is the intake/data coordinator
