Easter Seals – Nationwide, serving veterans since WWII Maine’s program began in 2012, 100% philanthropy-based Founded on proven NH model that combines Care Coordination + Emergency Financial Assistance Maine’s primary focus: Post-9/11 veterans facing barriers to successful transition to civilian life

6 Easter Seals’ Role in RVCP Grant
Expand Care Coordination statewide North of Bangor, Downeast Focus on recently separating veterans & their families (within 2 years of service separation) Work shoulder to shoulder with grant partners to help veterans navigate multiple support resources

7 Easter Seals’ Expected Grant Outcomes:
Care Coordination provided to veterans over the next 2 years Help avert suicide Link to appropriate resources for VA benefits, health & mental health care Refer to DOL, assist in job searches, college enrollment Aid in preventing homelessness

8 Easter Seals’ RVCP Grant Staffing:
Lois Jones, LCSW, supervises clinical aspects of the service statewide Care Coordinators: Ellen McGuire serves Bangor to Brunswick Sally Papciak serves Brunswick to Kittery Currently interviewing candidates for serving points north of Bangor Stacy Leet, MHRT/C, is the intake/data coordinator

Military & Veterans Program Care Coordination Key Outcomes Through June / N= 80 Homelessness prevented for 12 families 15% of all cases 54 referrals made to additional community resources No individuals required suicide intervention services
