2 W HAT IS S URVEY M ONKEY ? SurveyMonkey is an online survey generator created to help users create a quality survey. This tutorial will guide you through the process necessary for creating a survey.

4 Establish a goal - Have a goal for the survey. For example: Establish a goal - Have a goal for the survey. For example: If the survey is a post-course evaluation tool, the goal should reflect the desire to learn how to improve the course If the Survey is intended to determine what students know, establish a set of entry behaviors, and draft the survey to determine id those behaviors are present If the survey is a tool for data collection, ask questions about the specific topic

5 S URVEY B EST P RACTICES Strive for clarity - When conducting a survey, it is important to ask questions that have clear, concise answer choices. Instead of yes or no responses, offer a variety of choices that will provide relevant feedback Keep it simple – Keep the number of questions to a minimum. Ask questions that will provide the feedback that you need, and nothing more.

6 S URVEY B EST P RACTICES Allow for opinion – Use the essay question option to explain answers or add an opinion. This gives the respondent an opportunity to provide specific feedback to questions that Likert Scale does not provide. Review and Revise Have a colleague take the survey. Talk with them about your goals for the survey and if they survey, in their opinion, meets those gals Discuss the questions and responses to ensure that questions are understood Review response choices to determine if they effectively answer the survey question

7 T HE M Y S URVEYS P AGE Navigation, editing and management of Survey Monkey is accomplished through a series of drop-down menus, buttons and icons. The following section outlines the functions of these items

8 G ETTING S TARTED Paste this URL into your browser: http://www.surveymonkey.com/ http://www.surveymonkey.com/ This will open the Survey Monkey homepage. From here, you will create and administer all surveys.

9 G ETTING S TARTED To create the survey, you must log in. Type in the login information: Username UIW_SURVEY Password ASKME2DAY Check the Remember Me box Click Log In Now This opens the “My Surveys” page.

11 W ELCOME B ANNER B UTTONS Home – Returns to the login page Create Survey - Opens the New Survey creation Page My Surveys – Opens the My Surveys Page, this is a list of all of the surveys created in the Survey Account Address Book- A list of all of the email addresses in the Survey Pool (The UIW Email list) My Account – From here you can edit login information and preferences

12 M Y S URVEYS P AGE From My Surveys, you can also Search for surveys by title Manage surveys Locate specific folders Manage folders

14 S EARCH F OLDERS T o locate a folder, click on the Current Folder Dropdown Menu Click on the desired folder name

15 M ANAGE F OLDERS Click the Manage Folders button to manage, create, edit, add, assign, remove or search folders.

16 M ANAGING F OLDERS It is a good idea to create separate folders for each department, instructor, or class. Determine your survey preferences before developing your folders

17 A DD F OLDERS After the Organize Folders page opens, click on Add New Folder A new menu opens, type a name in the Folder Name box A new menu opens, type a name in the Folder Name box Click Add New Folder This will return to the Manage Folders page.

18 E DIT F OLDERS Click Edit Folders to open the Edit Folders List. To change the name of a folder, highlight and type a new folder name Click Save Name. When finished, click Back to Folder Manager

19 E DIT F OLDERS To remove a folder, click the Delete icon (The folder will be permanently deleted, surveys inside the folder will remain, but not assigned to a folder) When finished, click Back to Folder Manager

21 S EARCH S URVEYS T o search for a Survey, Type the survey title into the Tile search window Click Search. Survey monkey will locate and open the desired survey

22 M ANAGING S URVEYS There are five Survey Management options available by clicking the icons. These options should be managed by the survey author. Design: Click here to design the survey Collect: Once the survey has been completed, click on this icon to select options for sending the survey to recipients and collecting survey results Analyze: Click this icon to see the data displayed graphically Clear: Clicking this icon clears survey data Delete: Clicking this icon deletes a survey PERMANENTLY

24 C REATE S URVEY Choose a survey format: Copy Existing Template From Scratch New surveys are created From Scratch

25 C REATE S URVEY Type a title for the survey in the Enter a Title box Click Create Survey

26 C REATE S URVEY Choose a theme by clicking one of the Templates on the drop- down list. Click the New Theme button, and the Theme Editor will open. This will allow you to customize the existing theme templates. You may also choose the UIW Standard template

28 A DD Q UESTIONS & PAGES A new page opens with options to add a title and description. Name the Page Title “Question 1” Add a description if necessary

29 A DD Q UESTIONS Add Question There are two options to consider when adding questions. You can either set one question on each page, or you can set the entire survey on a single page. If you set up the survey with a question on each page, the Skip Logic feature may be used. Click Add Question Here

30 A DD Q UESTIONS Add Question list opens Choose a Question Type from the list of questions available including Multiple-choice,MatrixEssay Choose a question type by clicking on it

31 A DD Q UESTIONS The Add Question menu opens If you want the respondent to make a single choice of several possible answers, choose One Answer If you want the respondent to choose more than one answer, choose Multiple Answers. Choose horizontal or vertical depending on the question layout If you want students to ass a comment or short answer, choose the appropriate “open- ended” choice.

32 A DD Q UESTIONS Choose a Display Format Type the Question Text Type the Answer Choices Click Check Spelling to review the spelling of the answer Click Save Changes

33 A DD P AGES Using the instructions on the previous slides add a page and another question to that page Once the second page and question have been added, select Page 1

35 D ESIGN S URVEYS Once a Survey has been created, it can be edited and reviewed This process occurs in the My Surveys section Click on the Design Icon Alternatively, click on the name of the survey that was just created The survey page opens the Edit Mode

36 E DIT P AGES AND Q UESTIONS You may also access the edit functions by clicking the Edit Survey Button on the left side of the screen nd a series of buttons will appear. These buttons allow either the the page or the question to be edited In addition, there are options to move, copy, delete or add logic to pages or questions

37 E DIT P AGES Edit Page: Opens the Edit Page menu to allow you to edit the: Page Title Description When finished editing, click Save Page

38 E DIT P AGES Move Page: Adds two buttons (Cancel Move and Move Page Here) to the page to assist in helping you determine where the pages will be moved Copy Page: Adds two buttons (Cancel Copy and Paste Page Here) to the page to assist in helping you determine where the pages will be copied

39 E DIT P AGES Delete Page: If you determine that it is necessary to delete a page, there are three options: Delete all Questions Move all Questions to page above Move questions to Page below Choose an option and click Yes

40 S KIP L OGIC Add logic: Page skip logic (unconditional logic) will customize the path through your survey, without needing responses to questions. In other words, if you have a survey question that does not require an answer, the responder can skip to the next question. Question skip logic (conditional logic) requires that the question be answered. In addition, the answer to the question determines the next question. For example, If the question has a YES or NO answer, a YES answer can move to the responder to next question, and a NO answer can let the responder skip the next question. Do not use Question and Page skip logic on the same page

41 S KIP L OGIC When to Use Question Skip Logic: If you want to set up skip logic so that respondents will be asked some follow-up questions based on their response to a particular question, then you will want to place skip logic on a question. When you place skip logic on a question, it is generally a good idea to have this question as the last question on a survey page. Then you could place your follow-up question(s) on a separate page immediately following the current page. Your skip logic rule should be set up so that the respondents who do not choose the specific answer you are looking for will skip over the page of follow-up questions when they click “Next >>.”

42 S KIP L OGIC When to Use Page Skip Logic: If you have more than one group of follow-up questions and only want respondents to answer one group depending on their answer to a particular question, then you will want to create separate pages - one for each group of questions. This time, if respondents get sent to the first page of follow-up questions because of the way they answered your question, you do not want them to continue to the second page of follow-up questions. This is where you would want to place skip logic on the page. You could set up skip logic on this page so that when the respondents clicked “Next >>” they will skip over the other pages of follow-up questions and continue with the survey.

44 E DIT Q UESTIONS Edit Question: Opens the Edit Question menu to allow you to edit the Type of question, Question format, Question text Answer Choices Make necessary changes and click Save Changes

45 E DIT Q UESTIONS Move Question: Adds two buttons (Cancel Move and Move Question Here) to the page to assist in helping you determine where the questions will be moved Copy Question: Adds two buttons (Cancel Copy and Paste Question Here) to the page to assist in helping you determine where the questions will be copied

46 E DIT Q UESTIONS Delete Question: If you determine that it is necessary to delete a question, make choice and click Yes

47 E DIT Q UESTIONS Click Add New Logic Choose a Skip to question or page Click Add New Logic Click Save Logic

48 S URVEY O PTIONS Click Survey Options This opens the survey options page. Survey Options include: Page and Question Numbering: Check the boxes to add numbers to pages or questions Add Logo: Click the Browse button to locate and add a corporate or University logo to the survey Progress Bar Settings: Check the boxes to add a bar to show the survey respondents how much of the survey has been completed

49 S URVEY O PTIONS Survey/Page Titles: Check the boxes to add titles to the survey and/or each page Navigation Buttons: Highlight the text to change the titles of the navigation buttons Required Question Highlight: Check the boxes to add an asterisk to questions that respondents are requires to answer Once all choices have been made click Save Changes

50 P RINT S URVEY Click Print Survey Check the boxes to choose options for creating a printable copy of the survey Click Download Printable Survey to save the document as a PDF file

51 R ESTORE Q UESTIONS Click Restore Questions This page stores all deleted questions for later retrieval, if necessary

52 P REVIEW A S URVEY After all of the questions, pages, and options (Logic, Editing, Copy/Move) have been completed Click Preview Survey The survey opens in a separate window to allow you to take the survey to ensure that the Questions are accurate Spelling and grammar are correct Skip Logic options are correct

54 C OLLECT R ESULTS Collect - Click the Collect Icon to open the collect responses page. This page determines how your are going to send the survey to and receive the survey from respondents there are Three Options: Create a link to send in your own email message or to place on a webpage Upload your own emails and have us send a survey invitation Create a popup invitation for your webpage

55 C OLLECT R ESULTS Choose the create a link collection option and click Next Step

56 C OLLECTING R ESULTS If you already have a collection option chosen, Click on Web Link to open the Collect Responses tab

57 C OLLECTING R ESULTS This opens the Get Survey Link. There are 2 choices: Send Link in an Email: Provides a link that can be copied and pasted to an email. When the email is sent to a prospective respondent, The link is embedded in the email. The respondent reviews the instructions, then clicks on the link to open and take the survey

58 C OLLECTING R ESULTS Place Survey Link on a Webpage: Provides a link that can be copied and pasted on a webpage. The respondent is sent an email to direct them to the website where the survey is located The respondent reviews the instructions, then clicks on the link to open and take the survey

59 C OLLECTING R ESULTS Click Change Settings This page displays setting for collecting survey data: Multiple Responses: Choose if multiple responses will be allowed per computer Allow Responses to be Edited: Three choices: Respondents may not edit answers after submission Responses can be edited only after submission Responses can be edited any time during survey

60 C OLLECTING R ESULTS Display a "Thank You" Page: If you choose yes, add a thank you statement Survey Completion: When complete respondents are directed to another web page (i.e. www.uiwtx.edu) or the survey closes www.uiwtx.edu Save IP Address in Results: Stores respondent’s IP address (i.e. to send follow-up correspondence) Upon completion of the settings, click Save Settings

61 C OLLECTING R ESULTS Click Change Restrictions These functions restrict access to the survey including: Cutoff Date and Time for ending the survey Max Response Count: Limits the survey to a set number of responses Enable Password Protection: Adds a password to restrict access Enable IP Blocking to prevent users from certain IP addresses from completing the survey

62 C OLLECTING R ESULTS To activate the change restrictions, click the checkbox, and fill in the data as needed

63 C OLLECTING R ESULTS Click Manual Data Entry This page allows survey results to be entered manually. (i.e. entering a few responses collected from a paper version of the survey) Add a new response by clicking Add New Response This opens your survey. Take the survey according to the data gathered on paper

64 C OLLECTING R ESULTS Click Open Collector Now This opens the Open Survey link for respondents to take the survey. Open the survey and let a few colleagues take it (with password protection) to ensure the survey is giving you the data that you need Before you open the survey, be sure to add a “closed” message for when the survey ids no longer available

65 C OLLECTING R ESULTS Before you open the survey, be sure to add a “closed” message for when the survey is no longer available Click Update Closed Message Click Open Collector Now

66 C OLLECTING R ESULTS Note that the Open Collection Now button changes to Close Collection Now This will close the survey Another method of closing the survey is to click the My Surveys button Click on the Open Box to close the survey Remember that once you open the survey, you cannot edit the survey. The survey must be closed to enable the editing option.

67 C OLLECTING R ESULTS Opening the box launches the survey. Everyone who has access to the survey may take it. Once you open the survey, remember that you cannot edit the survey. The survey must be closed to enable the editing option. Click on the Closed Box to open the survey Click on the Open Box to close the survey

69 E MAIL /A DDRESSES Click Add Recipients On the next page choose an option Add Recipients from Address Book (See the Address Bock Section for adding an Address Book) Enter Recipients Manually If using an address book, choose an address book If manually, add the emails fin the space provided

70 E MAIL /M ESSAGE Click Create Email Message On the next page choose an option New/Unsent: Send to everyone who has not received an email Not Responded: Send to everyone who has received an email message, but has not responded Responded: Send to everyone who has responded All Emails: Send to everyone in your list, regardless of whether they have responded Custom Criteria: Send to everyone who matches the criteria you specify

71 E MAIL /A DDRESSES Click Save Selection and Continue Fill out the subject and message sections Add a Return Address Click Save and Preview

72 E MAIL /A DDRESSES Click Schedule Delivery to set up the date for delivery The next page requires you to fill out the survey agreement Click send message The survey will be sent on the specified date, or immediately as needed

74 P OP - UPS Choose a Pop up type Fill out the pop-up information Click Save Pop up Configuration On the next page that opens, preview the popup. Be sure to turn of the pop-up blocker

76 Click on the Analyze Icon This opens the View Summary Page This page contains the results of the survey, starting with Question 1 Click on the button to see the next question Click on View all pages to see the entire survey

78 Browse responses: to review survey responses by respondent Click the Previous or Next buttons Type a # in the Jump To box, then click Go
