2  All incidents involving Controlled Drugs must be reported to the CD Accountable Officer of the Area Team (Brigid Stacey) as soon as the incident is identified.  Team to support Brigid in this role; ◦ Helen Pearce – Clinical Intelligence Co-ordinator ◦ Eleanor Carnegie – CD Support Officer ◦ Alison Boulton CD Support Officer ◦ Andrew Pickard – Pharmacy Advisor ◦ Incident forms and the flowchart can be requested directly from the Area Team, or downloaded from the CD section of the North Staffs and Stoke LPC website

9  104 incidents community pharmacy related  Of these, 53 involved Methadone/Buprenorphine  Approx 30% involved the issuing of Sugared/S.Free Methadone when prescription for the other preparation  Several incidents where the instalment instructions have not been followed correctly (ie. Two weeks supply given on a single occasion when script requested 7 day supply only)  Issuing without a legally valid prescription – prescription from previous week not put through, and then dispensed against

10  Always check the validity of the prescription immediately upon receipt.  Double check instalment intervals, and ensure quantities correlate with total amount requested  Always get a second check both during the dispensing process, and before handing out  Clearly segregate SF and Sugared Methadone in CD cabinet, and highlight label if necessary  Take your time – avoid other distractions
