1 Scissor Lift Awareness Stand Down Training May 4-10, 2014 New England Safety Training Alliance is a sub-committee of the Construction Safety Roundtable of Eastern Massachusetts OSHA Alliance dedicated to providing quality training products free of charge.

5 Mobile Scaffolding Accidents A worker on a self-propelled scissor lift was installing bolts to connect two large overhead steel beams. The platform controls were not protected against inadvertent operation. The worker inadvertently activated the lever for controlling elevation. The platform elevated. The worker was pushed firmly onto the control lever when he was caught between an overhead beam and the elevating platform’s guard rail. The platform continued to rise, fatally crushing the worker.What should be done to prevent a similar incident?Use only elevating work platforms with controls that are protected against inadvertent operation!
6 Mobile Scaffolding Accidents A window washer was killed when the elevated scissor lift he was operating flopped sideways, throwing him to the pavement below. He died shortly afterward from massive head and back injuries. At the time of the accident, the window washer was repositioning the self-propelled scissor lift. The lift was equipped with an operational tilt sensor that automatically lowers the operator's lift platform when the tilt exceeds three degrees to any side. He was operating the lift on an eight-degree slope, exceeding the slope capacity by five degrees. However, the wires leading to the tilt sensor had been intentionally disconnected, leaving the window washer without any tilt protection.What should be done to prevent a similar incident?Conduct routine equipment inspectionsVerify functionality of all safety devicesDo not override safety devices

9 UNSAFE CONDITIONS Workplace Inspection Before and during using the scissor platform, the operator shallcheck the area in which the lift is to be used for possible hazardssuch as, but not limited to:Drop offs or holes.Slopes., Bumps or floor obstructions.Debris.Overhead obstructions and electrical conductors.Hazardous locations.Inadequate surface and support to withstand all load forces imposed by the scissor lift in all operating configurations.Wind and weather conditions.Contact with ceiling and overhead objects.Close proximately of workers.
10 Work area inspectionThe surface of the scissor lift being moved shall be within 3 degrees of level. Speed of scissor lift shall not exceed 1 foot per second.3° max
11 Work Area Inspections Look for: Untamped earth fills Contractor Insert own photo for discussionLook for:Untamped earth fillsDitches, drop-offs, or holesBumps and floor obstructionsDebrisOverhead obstructions and high-voltage conductorsOutrigger extensions used if applicableContractor can insert custom site photo hereSlide Show NotesAfter inspecting the equipment, you need to inspect the immediate work area for hazards that could result in an accident such as a fall, tipover, or electrocution.Check for any recent earth fills that may not have been properly compacted. If a tire or stabilizer is placed on the soft earth, the equipment may tip over when the boom is lifted or extended.Keep a safe distance from ditches, drop-offs, or holes to avoid accidentally driving into one, which could cause a tipover. If you use equipment too close to these areas, the side of a hole or ditch could give way under the weight of a tire or stabilizer.Watch for bumps and other floor obstructions. Be careful not to drive over these obstructions because they can impact stability.Look out for debris and other things such as grease or water that could result in loss of stability.Finally, be especially careful of overhead obstructions and high-voltage conductors. Look for them and remember where they. If there are any overhead obstructions, consider having a spotter on the ground who watches as the operator moves the aerial lift, and be sure the spotter has an effective way to communicate with the lift where they are.What’s the ladder for?
12 Safe Work Practices Keep at least 10 feet away from power lines Report defects or malfunctions immediatelyNever disable a safety device or interlockPrevent ropes, cords, and hoses from entangling with liftBefore lowering the lift make sure the area underneath is clearNever engage in stunt driving or horseplaySlide Show NotesHere are some safe work practices that all aerial lift operators and other employees working with them should observe. We’ve touched on some of these already, but a little review never hurts.Check for overhead power lines and high-voltage conductors. Keep the lift and boom at least 10 feet away from power lines at all times, unless you are specifically working on the lines.Report any defects or equipment malfunctions immediately, and do not use the equipment until necessary repairs have been made by qualified maintenance personnel.Never alter or disable a safety device or interlock device. And be sure to check safety and interlock devices daily to ensure they are functioning properly.Prevent ropes, electric cords, and hoses from entangling with the lift when the platform is elevated, lowered, or moved.Before lowering the lift, make sure the area under the lift is clear of tools, debris, or personnel working on the ground.And, of course, it should go without saying that you should never engage in any stunt driving or horseplay when working in or around aerial lifts. Such behavior is always unsafe and prohibited.
13 Fall HazardsLeaning or climbing over scaffold guardrails could result in a fall off the scaffoldingHorseplay on a scaffold could have serious consequencesIce, snow, rain and liquids on platform can cause slips/possible fall….keep as clear/dry as possibleSlips or trips on equipment, tools, etc., used on the scaffold platform. Keep it tidy!Do not stand on stacked material to get additional height/reach
14 Fatal scissor lift accident – Ma The guardrails on this machine are designed to be lowered/stowed to fit through a standard size door opening. The rails were lowered/stowed to move the lift into the next room and not raised to the specified height before being put into service.
15 Fall HazardsFall Protection is normally not required for scissor lifts butmany local general contractors go above and beyond theOSHA regulations and do require it.Guardrails must remain up.Do not lower or stand on guardrailsDo not use ladder or any other object to gain height.
16 Can I exit an elevated scissor lift? CFR Subpart “L” has a 10’ Fall protection trigger height. Scissor lifts are scaffolds and can be exited as long as “100% Fall Protection” is maintained at all time over 10’.After employee has exited the equipment the 6’ fall rule may apply.
17 The 6’ fall requirement under CFR 1926 Subpart M applies after you have exited the lift. The employee is exposed to fall hazards.
18 Standing on the rails to gain extra height is NOT allowed.
19 Falling Object Hazards Tools, equipment, materials, ice, etc. can fall from work activities on platform, potentially injuring personnel below Hardhat required for all users on, and persons working below, scaffold Place tools and other equipment or materials away from edge of platform Protective zones below scaffold also protect workers from falling objects
21 Electrical HazardsElectricity has the power to shock, burn, and kill; not to mention cause fires or explosionsElectrical hazards encountered on a scaffold can be from power tools and equipmentMisused or damaged extension cordsContacting overhead power lines- Always stay at least 10 feet away!
22 Electrical SafetyBecause the metal frame of scissor lifts are conductive, power tools, cords, etc., that suffer insulation failure can electrify the entire lift. This poses a risk of electrocution not just to the worker holding the tool, but also to everyone who contacts the scaffold. GFCI (ground-fault circuit interrupters) are required work using electrically powered equipment on construction sites and outside or in wet or damp locations.
23 Power CordsAll power cords used on scaffolding will be properly grounded and inspected prior to and during work, and will be approved for outdoor or wet conditions when applicable Do not tie cords to lifts, a snagged cord can cause the lift to tip over Do not hang equipment on power cords/lines Never mix/match 2 prong power/extension cords with 3 prong tools/equipment Do not set equipment on power cords where they could cut/abrade them Do not place cords where they could become a tripping hazard
24 Electrical HazardsDanger ZoneIf a fellow worker does come in contact with an energized power source, do not rush to assist them. Immediately call 911, try to shut off the main power source, and follow the directions of the emergency rescue medical personnel
25 Wind HazardsWind is an additional concern of any aerial/scissor lift operator who operates outdoors. How does wind effect the lift?Creates additional force on the platform.Does not actually add any additional weight, only applied force, however your machine does not know the difference.Wind can be different above roof levels.Additional force caused by the wind can cause tip over.Follow the manufacturer’s rules for operating in wind.See OSHA Hazard Alert regarding Notre Dame accident
26 Other Scaffold Hazards: Getting Crushed/PinchedA dangerous aspect of working with a scissor lift is the potentialis having a limb or other body part pinchedor crushed in the scissor arms or betweenthe top rail and overhead structureThese arms collapse down upon each other at multiple joint locations when the platform is lowered, all of which serve as entryways for potential injuryEnsure command of platform controls at all times to avoid “over-lfting”NEVER put your hands or feet under the platform of the scissor lift unless you need to perform maintenance. In which case, you will need to disconnect the power and insert safety bars, which prevent the platform from collapsing.

28 Scissor Lift Operator Safety Training Before OperationBefore operation the operator shall: Read and understand the manufacturer’s operating instructions and user’s safety rules.Understand all labels, warnings and instructions.Understand ANSI standards
30 Trained and Authorized Operators Only operate scissor lifts if you are trained and authorizedUnderstand operating instructions and safety rulesReceive hands-on training from a qualified personKnow how to inspect equipment and work areaDemonstrate how to safely operate equipmentSlide Show NotesOnly trained and authorized employees may operate equipment with aerial lifts. Authorized operators must read and understand the manufacturer’s manual for the specific equipment to be operated. Operators must pay particular attention to operating instructions and safety rules.In addition to reviewing the operating manual, operator trainees should receive hands-on training from a qualified person who has experience using the specific equipment to be operated and can judge the trainee’s proficiency using the equipment.Operators must be trained to inspect the equipment and work area before each use. They must know what to look for and what to do if they discover something that is not safe.Operators must also demonstrate that they can proficiently and safely operate the equipment, including following all safe work practices and safety rules.Think about the training and proficiency requirements for aerial lift operators. Remember, you may only operate this equipment if you are trained and authorized.Discuss training requirements for aerial lift operators.
31 Scissor Lift Operator Safety Training Full Functional Test Prior to the beginning of each shift the operator shall perform a full functional test on all machine controls starting with the lower controls first. Problems or Malfunctions Any problems or malfunctions noted that affect safety and/or operation shall be repaired prior to use of the lift platform
33 Pre-Inspection Pre-Start Inspection Before each day or at the beginning of each shift, the scissor lift shall be given a visual inspection including but not limited to the following:Operating and emergency controlsSafety devicesPersonal protective devicesAir, hydraulic, and fuel systems leaksCables and wiring harnessesLoose or missing partsTires and wheelsPlacards, warnings, control markings, and operating manuals (must be legible)Outriggers, stabilizers and other structuresGuardrail systemOther items specified by the manufacturer
34 Equipment Inspections Look for:Cracked weldsHydraulic leaksDamaged control cablesLoose wire connectionsTire damageCheck electrically insulated itemsPerform a control check and lift testCorrect unsafe items before using liftSlide Show NotesAerial lift equipment must be inspected before each use. Look for such defects as:Cracked welds;Hydraulic leaks;Damaged control cables;Loose wire connections; andTire damage.Also check electrically insulated items and make sure they are all there and in good condition.And finally, perform a control check to make sure operating controls are all functioning correctly, and perform an operating test on the lift itself.Any potentially unsafe items discovered through inspection must be carefully examined and a determination must be made by a qualified service person as to whether the problem constitutes a safety hazard. All unsafe items and conditions must be corrected before the aerial lift can be used.Think about the inspection procedure you need to perform before using an aerial lift.Review your safety inspection checklist for aerial lifts.
