4 Individual folder for each student athlete.

5 Birth Certificates Birth date must not be prior to July 1, 1994

6 Foreign Birth Certificates Must include a completed verification form

7 Grades Transcript To be eligible during the second semester the student must have an overall passing average and meet one of the following conditions: a. If the student met first semester eligibility requirements then he or she must pass the equivalent of four, ½ units during the first semester. b.If the student did not meet first semester eligibility requirements then he or she must pass the equivalent of five, ½ units during the first semester. In most cases on a traditional or AB block schedule, the following example would apply: If eligible first semester, must pass four subjects If not eligible first semester, must pass five subjects In a 4 X 4 block schedule where units or ½ units are granted at the end of the first semester the following will apply: If eligible first semester, must earn 2 units If not eligible first semester, must earn 2 ½ units

9 Non-Diploma Students Meeting IEP Verification (no transcript or report card necessary) –Letter from principal indicating student is non-diploma student and meeting IEP

10 Parent permission signed & dated by parent & student on or after April 1, 2013

11 Physical Form signed by the appropriate health care professional on or after April 1, 2013

12 Home School Proof of residency in attendance area Verification of one year in home school

13 Charter Students Proof of residency in attendance area

14 If Yes in transfer column on eligibility, check for list of received transfers to verify Special A form has been submitted

15 Hardship Transfer

16 All Foreign Exchanges students must me approved by SCHSL. Check for list of approved foreign exchange

18 Notice of Possible Violation(s)

19 Tips for Success You will receive a call at least two days prior to audit visit. Please have all information be located at a central site (i.e. one room). Please have an individual folder for each athlete with all necessary information. Allow at least 1 ½ hours for school visit. You will be provided with a copy of all signed forms (Audit record sheet & Possible Violation Notice ) if necessary. Compliance Team Members will not make any rulings on “possible violations”. Report any concerns to League office.
