Steps to Completing the GEN 1321 (WIS ACCESS) Section 1, page 1 Check ONLY ONE of the boxes. You are either a brand new WIS user – Check “New Access” box. You need to terminate your or an employee’s access – Check “Terminate Access” box. Everything else – Check “Other” and explain, e.g. “Reset password,” “Restore user access.” On the “Current Bureau/Unit/Index” line be sure to write your county as well as any other appropriate information, e.g. “Inyo Co. DHHS.” Do not complete any other portions of Section 1. Section 2, page 1 In “USER NAME” portion write your last and first name as well as MI if needed. If you are simply changing your name – check the “Check if name change” box and complete this portion with your new name, then beneath in the “FORMER NAME” portion write your old name. In the “ADDRESS,” “CITY,” “STATE,” and “ZIP CODE,” portions write your mailing address. In the “PHONE NUMBER” portion, if you have a direct phone number, use that phone number. If your phone message identifies you as the person to whom that line belongs, the staff assigning logon IDs can leave your password on your voice mail. This is extremely useful. Include “FAX NUMBER” and “E-MAIL ADDRESS.”
