Coordination of Care All MH services were provided at HOPE – Counseling, Collateral (parent education), Psychiatry, and TBS Working with outside agencies to utilize strengths and build upon them by having a meeting every 4-6 weeks during the duration of TBS – Able to pull skills from different settings to create a streamlined plan that worked for individual

12. Results Behaviors were decreased to: 0 incidents of aggression, increasing compliance in the home and school settings, and reducing AWOL to 0. Support system: Family, teacher’s aide, and extended family. Transition plan: continuing counseling and collateral support at HOPE. Starting an afterschool program and recreational activities.
