New S+C and S8 MR SRO applicants should fill out: Section I Parts A through G Section II Part J Section III

6 PERMANENT HOUSING BONUS
The Permanent Housing Bonus, formerly known as the Samaritan Bonus, will be available to create one or more new permanent housing projects. Proposers may use project funds to serve homeless and disabled individuals or families, or chronically homeless individuals. While proposers are no longer required to use these funds for projects that will exclusively serve 100 percent chronically homeless individuals, proposers are encouraged to use bonus funds for this population whenever possible, as creating new beds for the chronically homeless remains a HUD priority. Permanent Housing Bonus projects may be Shelter Plus Care or Moderate Rehabilitation SRO projects.

All applicants skip chart B1 All S+C applicants complete chart B2 Select only one component Total rental assistance amount is calculated in Section II, Part J, chart J1 and carried over to Sec. I, chart B2

S8 MR SRO applicants complete chart B3 HUD website for FMR Total rental assistance amount is calculated in Section II, Part J, chart J1 and carried over to Sec. I, chart B3

10 Section I, Part C: Point in Time Housing & Participants Chart
All applicants must fill out New S+C and S8 MR SRO applicants should insert numbers only under column b, New Effort or Change in Effort

11 Section I: Parts D through G
Complete Part D Disregard Part E Part F: Project Leveraging Chart Include only services for which there is a written commitment If applicable, complete Part G

12 Section I, Part H: Renewal Performance
Renewal are not included in this RFP.

13 Exhibit 2, Section II: Project Budgets
All applicants disregard Part I and go to Part J All applicants should complete chart J1 Only new S+C SRO applicants and S8 MR SRO applicants complete chart J2
