7 7/16/08 New Mexico Department of Health 7 Community Health Assessment Program (CHAP)  Core Activities: –Provide training and technical assistance to DOH programs, counties, tribes and other local entities in the use and interpretation of public health data. –Maintain Web-based access to meaningful data and information to support community assessment activities. –Conduct population-based assessments relevant to local needs. –Develop methods to provide meaningful data for sub-county New Mexico geographies where population size permits it. –Evaluate the impacts of locally-conducted community health assessment on program and policy at the local level and within state health and human services agencies.
