1 Etiology Clostridium tetani – Gram (+) spore forming bacilli – a rod-shaped, resembling tennis rackets or drumsticks. – Tentanospasmin LOPEZ, CAMILLE CECILIA

2 Pathogenesis Contamination of wounds with spores of C. Tetani Tetanospamin binds to presynaptic membranes of motor neurons Migrates to spinal cord & brain stem Toxin diffuses to terminals of inhibitory cells & degrades synaptobrevin Release of glycine & GABA is blocked, and the motor neurons are not inhibited LOPEZ, CAMILLE CECILIA

3 CLINICAL MANIFESTATION Generalized tetanus – Most common form – Increased muscle tone and generalized spasms – Initially presents with lockjaw or increased tone in the masseter muscles – Progresses to dysphagia, stiffness or pain in the neck, shoulder, and back muscles – Hands and feet are spared – Constant threat of reduced ventilation or laryngospasm LOPEZ, CAMILLE CECILIA

4 CLINICAL MANIFESTATION Neonatal tetanus – Children born to inadequately immunized mothers – Poor feeding, rigidity, spasms – Fatal if untreated Local tetanus – Uncommon; restricted to muscles around wound Celphalic tetanus – Rare form that proceeds an ear infection or head injury – Dysfuntion of 7 th CN – High mortality LOPEZ, CAMILLE CECILIA
