What is happening locally? Trying to grow Triple P through: keeping in mind population health strengthening relationships with community partners media strategies, normalizing schools, daycares and health care providers improving visibility of Robyn in her role working to facilitate transfer of care

2011/12 Statistics Total number of families referred to Triple P: 555 * (target, last year’s number) (495, 387) Total number of families referred by CAS: 68 (100, 56) Number of families served – CAS referrals: 45 (75, 44) Number of children served – CAS referrals: 101 (160, 95)

Grant Updates – Measurable Outputs Item2 year target YTD Total Description School Parent Councils Attended 3218 + 3 individual Elementary schools, one meeting, recruiting members for Triple P Parent Advisory Council. -All elementary schools in both UCDSB & CDSBEO contacted -18 requested presentations at parent council meeting – completed -10 schools did not respond to requests -3 meetings are in the process of being scheduled -2 schools did not wish to receive parent council presentation, but requested TP blurb/advertisement for newsletter -Individual meetings were also held with 3 secondary school principals
Grant Updates – cont’d Item2 year target YTD Total Description PowerPoint presentation for school councils 11PPT describing Triple P, the community initiative and the role of the parent advisory council Presentation includes: program overview, historical context, global perspective, community initiative, role of FTE practitioner, referral information, etc. Parent members for Triple P Parent Advisory Council identified 113One from each municipality 3 individuals have expressed interest in sitting on the Council. These individuals have been provided with details regarding the goals of the Council, as well as the first meeting, May 28th.
Grant Updates – cont’d Item2 year target YTD Total Description Triple P Parent Advisory Council Meetings 81 planned 2012- 4 Mtgs, 2013 – 4 Mtgs Individuals who have expressed interest in sitting on the Parent Advisory Council have been contacted and the first Council meeting has been scheduled for late May 2012. Job description for one FTE Triple P level 3 practitioner position 11Job description describing the experience/background required, responsibilities and wages of the FTE, 1 year term practitioner position. A job description was created which outlines: background of the position, description of duties, qualifications (both required and preferred) and wages.
Grant Updates – cont’d Item2 year target YTD Total Description One FTE for service provision Job description for one FTE Triple P level 3 practitioner position 24036 referrals 29 received service Families receive Level 3 Primary Care 0-12 and Teen Of the 36 referrals: -7 clients/families have completed -16 clients/families are currently engaged -6 clients/families engaged but did not complete -7 clients/families did not engage in services In addition, 65 parents were served through presentations/discussion groups at OEYC sites, Baby Talk, and Good Food for a Healthy Baby.
Grant Updates – cont’d Item2 year target YTD Total Description Number of tip sheets distributed 10001374Tip sheets given to families during intervention Tip Sheets are tracked via order form by the Leeds, Grenville and Lanark District Health Unit. As of March 31, 2012 – 1374 tip sheets had been ordered and picked up by community practitioners for use during interventions with families.

Other 2 project plans are to be complete A Community Plan was compiled and included plans for the following target sectors: -Education -Daycares / Early Childhood Centres -CHCs/FHT -Workplaces Number of new partners 55 Schools, daycares, HCPs, volunteers -2 schools (seminar series) -1 daycare (15 staff members trained) -2 community volunteers (trained)

Results from the evaluations will be available during the 2 nd year of the grant.
Other Grant Details… Have not addressed physical activity, obesity No training availability, Need partners. Need more parent representation on advisory group Need more partners who can provide services for parents of teens
