Form-Service Mapping The following fields from the US2014 form are mapped to a Vaccination medical service: – Vaccine Given by Panel Site Date – Additional Vaccine Given by IOM Date 1 – Additional Vaccine Given by IOM Date 2 All 3 date fields populate the Completion Date of the Vaccination medical service If all 3 date fields are populated, the Health Assessment of the individual will show 3 instances of the vaccination medical service ND is the Not Done checkbox field that sets the status of a core Vaccination medical service to Not Done
