Demonstrating SR eligibility/capacity Criteria for assessment (county level applicants) 1Organization is legally registered 2Technical capacities to plan, organize, implement, monitor and report on projects through a participatory approach (as evidenced by reported qualifications of key staff and previous project reports) 3Evidence of audited accounts for the last three years 4Evidence of financial management capacity and systems and internal controls and bank details

Demonstrating SR eligibility/capacity Criteria for assessment (county level applicants) 5Evidence of a board of directors and governance system (attached 2 recent board meeting minutes) 6Application process fully complied with 7Evidence of implementation in HIV or TB or Malaria for at least 2 years 8Linkages to communities in county in which activities will be implemented

(YES or NO) Was the proposal submitted in the appropriate place (YES or NO) Hard and soft copy? (YES or NO) Was the proposal delivered in the right format? (YES or NO) Are all the attachments provided? (YES or NO) Qualify (if yes in all categories) Disqualify (if no in any category)
