2 United States Coast Guard SIGNIFICANT INCIDENTS F/V Josias 16 July at the entrance to the Port of Brookings Harbor $200,000 of OSLTF used 03 personnel onboard pulled off by Coast Guard crew 300 gallons onboard Vessel floated off rocks with airbags and craned out of water

3 United States Coast Guard SIGNIFICANT INCIDENTS F/V New York One mile north of the Coos Bay North Jetty $20,000 of OSLTF used 03 personnel onboard swam to shore through surf zone 200 gallons onboard Vessel is total constructive loss Fuel tanks and debris removed from beach by owner

4 United States Coast Guard SIGNIFICANT INCIDENTS M/V EDFU Oct 11: Lost propulsion 9NM off the mouth of the Columbia River Sector prepared for ICP stand-up Vessel able to anchor and tugs on- scene within 12 hours Able to make repairs verified by Class Society and proceeded into port Nov 10: Lost propulsion transiting outbound IVO Clatsop Spit Tug WILLAMETTE CHAMPION directed to drop load and assist the vessel Issued COTP Order requiring appropriate sea going tugs to be on station and tend to the vessel while transiting in the Columbia River COTP Zone off the coast of Oregon and Washington, in U.S waters Required Class Society to verify satisfactory repairs and proper operation of the main engine and anchors Coordinated with Sector Puget Sound for transit plan and requirements

5 United States Coast Guard SIGNIFICANT INCIDENTS M/V ARION SB Report of sheen at Terminal 4 IVO Vessel Pollution Investigators found extra pipe on suction of bilge pump to cooling water system not on vessel diagram Port State Control and CGIS conducted follow-up exam and discrepancies were found in the operation of the OWS and Oil Record Book entries Case forwarded to Dept of Justice and US Attorney for prosecution of violating Act to Prevent Pollution from Ships Nov 2: Plea agreement reached and $750K Fine Assessed

6 United States Coast Guard LST 1166 COTP provided EPA with Pollution Removal Funding Authorization providing reimbursement to conduct disposal alternatives assessment and generate Engineering Evaluation/Cost Analysis report on for the LST EPA completed EECA and Sector CR is reviewing findings
