8 Who should be reporting?  Health care providers and laboratories are required under 105 CMR to report anaplasmosis and babesiosis cases What should be reported?  Any healthcare provider diagnosed case of disease Anaplasmosis and Babesiosis Disease Surveillance

9 How is data collected? Physician sees a patient Physician suspects disease Lab test ordered and is positive MDPH-SLI LBOH

16 Who should be reporting? HHealth care providers and laboratories are required under 105 CMR to report Lyme disease cases What should be reported? AAny healthcare provider diagnosed case of Lyme disease based on clinical signs and symptoms regardless of whether or not laboratory confirmation testing was done Lyme Disease Surveillance

18 Where should a physician report a patient who has Lyme disease?  to the MDPH Office of Integrated Surveillance and Informatics program via confidential fax at When should Lyme disease be reported?  As soon as possible after a diagnosis is made. Lyme Disease Surveillance

19 How is data collected? Physician sees a patient MDPH-SLI Teleform completed Teleform generated and sent to physician Physician diagnoses Lyme disease Lab test ordered and is positive LBOH
