Emergency Management Division

3 3  SLED Application Guidelines and Instructions incorporated with ODP Guidelines for Authorized Equipment Lists, Allowable Costs and Guidance for Requesting ODP Approval of Non-ODP Course Development and Attendance  Sample Grant Application—Budget Worksheets, Program Narrative and Signature Pages  State Strategy Goals and Objectives  County SHSP and LETPP Allocations—amount does not include COBRA or EOC Allocations

4 4  Instructions for Completion of Budget Worksheets—by Category, Discipline and Objective  More than Two Objectives Requires Additional Budget Worksheets  Organization Worksheet for LETPP Grant Only  Budget Narrative for Each Budget Worksheet  M&A allowable for local jurisdictions only— cannot exceed 2.5% of total grant award

5 5  Submit Original Signed Application by mail and one Electronic Copy, including Budget Worksheets and Program Narrative, to  Signatures of all County Needs Assessment Committee Members to be considered complete  Due No Later Than Close of Business March 11, 2005
